Specific Criteria Needing To Be Met for Medical Cannabis Card in Florida:

1. Residency and Age Requirements:

a. You must be a legal Florida resident with a valid driver’s license

b. Or must be a seasonal Florida resident meaning, you must stay in Florida at least 31 consecutive days of a calendar year and maintain a temporary residence in Florida

c. MUST be 18 years of age, if you are under the age of 18 you MUST HAVE parental consent

2. Not required but preferred, qualifying Pre-Existing Medical Condition with failed treatment(s), some examples include:

a. Cancer

b. Epilepsy

c. Glaucoma

d. HIV/AIDS

e. Post-Traumatic Stress Disorder (PTSD)

f. Amyotrophic Lateral Sclerosis (ALS)

g. Crohn’s Disease

h. Parkinson’s Disease

i. Multiple Sclerosis

j. Chronic Non-Malignant Pain

k. Depression and Anxiety

l. Rheumatoid Arthritis

m. Alzheimer’s Disease

n. Autism

o. Terminal Conditions diagnosed by a physician other than the qualified physician issuing the certification

3. Physician Certification:

a. Receive an in-person recommendation from qualified physician, who is registered in the Florida Medical Ma*****na Use Registry

b. Physician will evaluate and assess your conditions and give recommendation for medical cannabis use based on evaluation and assessments

4. The Application Process:

a. Schedule an appointment/consultation with a qualified physician

b. AFTER certification, complete your application in the Florida Medical Ma*****na Use Registry, providing proof of Florida residency, and paying state fees
