As with every profession, physical therapists have different treatment philosophies. I rarely start my evaluation with an internal examination.

There are many pelvic physical therapists that I know and respect deeply that would argue that the internal examination and treatment is the most important part of what we do in this specialty and should be prioritized first. I haven’t found this to be true in my own practice, but that doesn’t mean that I am claiming that I am right and they are wrong. Only that I may be a better fit for someone like the woman who posted that video.

The pelvic floor muscles clench when we are anxious. So if a patient is anxious about what to expect on day one, and I find pelvic floor tension when I do an exam - is this an accurate indicator of what the muscles are doing on any given Tuesday? Maybe.

1. If you want to go to a pelvic physical therapist but do not know what to expect - call and ask. Make sure it is a good fit.

2. The pelvic floor examination gives us great information about what exactly the pelvic floor is doing. It is a great tool that helps to guide treatment. (Also - this exam should not be painful. I will do another post on this later.)

3. You can absolutely make progress without having this be a part of your plan.

4. Make sure that your pelvic PT isn’t only focusing on your pelvic floor, but how functions with the rest of your body.

When I watch someone actually DO THE THING they are struggling with - I learn so much that I cannot see when my patient is on a treatment table.

If I don’t watch someone move, I might totally miss that they suck in their abs while they run, shift to one side when they squat, or hold their breath when they jump.

This may or may not be a problem (it’s not a problem if it’s not a problem) - but soooo many times making a small adjustment to a movement pattern can make all the difference. For example, I recently asked a patient to “push through her feet” instead of thinking of pulling the bar up during a power clean - and her technique and symptoms immediately improved.

This movement analysis applies to my pregnant patients as well! There are definitely times when women should modify their exercise routine during pregnancy, and many more times when they just need to right guidance on how to continue what they are doing while accounting for the changes in their body.

If it isn’t for a good medical reason - please do not let anyone discourage you from exercise during your pregnancy. It is not only safe - but encouraged for your health, baby’s health, and improved postpartum recovery.
