Persistent fatigue can have many causes, including medical conditions, chronic stress, anxiety, burnout, and other life stressors. If you are experiencing ongoing exhaustion that is affecting your daily functioning, it may be beneficial to consult with your healthcare provider to rule out potential medical concerns.

When emotional stress, anxiety, or burnout are contributing factors, counseling can provide support, coping strategies, and a space to address the underlying issues that may be impacting your overall well-being.

If Gretchen Wieners were a therapy client, today’s presenting concern might be:

“Loss of interest in previously enjoyed activities.”

Normally, cucumber is a highly preferred activity. Today? She’s just lying there staring into the distance contemplating her life choices.

In all seriousness, one sign that something may be wrong is when we stop enjoying the things that usually bring us comfort, happiness, or fulfillment. Sometimes it’s stress. Sometimes it’s burnout. Sometimes it’s depression.

Pay attention when the things you love no longer feel like enough. It’s often your mind’s way of telling you that something needs attention.

(For the record, Gretchen recovered fully and eventually ate the cucumber. 🥒🐾)

Today was my first half day working from my new office space and I had so much fun! 🖤

Since I wasn’t home, Gretchen Wieners spent the day at work with her dad at the tractor dealership — which honestly isn’t unusual for her. 😂 She’s basically part of the staff at this point.

Over the next few weeks, she may begin making occasional in-person appearances during psychotherapy sessions for clients who enjoy dogs. 🐾

Current qualifications include:
• Excellent listening skills
• Strong emotional support presence
• Supervising snack breaks
• Frequent visitor at the tractor dealership
• Accepts shrimp from Carlos’ Street Tacos as payment 🍤

I have a feeling she’s going to love office life.

# # Questions That May Surprise You During Your First Psychiatric Diagnostic Evaluation 🛋️✨

Starting therapy can feel intimidating enough — and then suddenly you’re being asked questions that seem random, awkward, deeply personal, or unrelated to why you scheduled the appointment 😅

Many psychiatric diagnostic evaluations are designed to help therapists understand the *whole person*, not just the immediate concern that brought someone in. Mental health is connected to relationships, physical health, stress, trauma history, sleep, work, culture, safety, and daily functioning. 🧠💭

Some questions may feel uncomfortable or surprisingly direct, but there is usually a clinical reason behind them. Psychiatric diagnostic evaluations also help therapists:

* 📋 gather background information
* 🛡️ assess safety concerns
* 🧩 identify patterns
* 🧠 understand symptoms more accurately
* 🤝 build an appropriate treatment plan
* ⚖️ meet legal, ethical, and insurance documentation requirements

Here are some questions that commonly surprise new clients — and *why therapists ask them*: 👇

---

# # # 🩹 “Do you have any history of trauma?”

This isn’t asked to force disclosure or make someone relive painful experiences. Trauma can impact:

* 😟 anxiety
* 😔 depression
* 💤 sleep
* ❤️ relationships
* 🤕 physical symptoms
* 🌊 emotional regulation
* 🚩 hypervigilance or panic responses

Sometimes clients do not initially realize certain experiences “count” as trauma because they normalized them growing up.

Understanding trauma history helps therapists choose appropriate treatment approaches and avoid moving too quickly emotionally.

---

# # # 👶 “What was your childhood like?”

Many people expect therapy to focus only on current stressors. Childhood experiences can influence:

* 🔗 attachment styles
* 🗣️ communication patterns
* 💔 self-esteem
* 🚧 boundaries
* 🛟 coping skills
* ⚡ conflict responses
* 🧍 relationship expectations

This question is not about blaming parents for everything. It helps therapists understand long-standing emotional patterns and learned survival strategies.

---

# # # 🍷 “Do you use alcohol, ni****ne, ma*****na, or other substances?”

Clients sometimes become defensive because they assume they are being judged or screened for addiction. In reality, therapists ask because substances can affect:

* 😵 mood
* 💤 sleep
* 😰 anxiety
* 💊 medication effectiveness
* 🌪️ emotional regulation
* 🧠 concentration and memory

Sometimes people are using substances to self-medicate stress, trauma, insomnia, panic attacks, or emotional pain.

---

# # # 🚨 “Have you ever had thoughts of hurting yourself?”

This question can feel alarming to first-time clients, but it is a standard safety assessment during a psychiatric diagnostic evaluation. Therapists ask to:

* 🛡️ assess current risk
* 🧠 understand emotional distress
* 📉 evaluate hopelessness levels
* 🤝 provide appropriate support and resources

Being asked does *not* mean the therapist assumes someone is suicidal.

---

# # # 😳 “Have you ever been abused?”

This may include:

* physical abuse
* emotional abuse
* sexual abuse
* neglect
* domestic violence
* coercive control

Clients are often surprised by how directly this is asked. Abuse histories can significantly affect:

* trust
* relationships
* self-worth
* nervous system functioning
* emotional regulation

You do not have to disclose details before you feel ready.

---

# # # 🫂 “What is your support system like?”

Therapists often ask about:

* 👨‍👩‍👧 family
* 🧍 friendships
* ❤️ partners
* ⛪ community involvement
* 🌧️ loneliness or isolation

Support systems can strongly affect emotional wellbeing, resilience, recovery, and crisis risk.

---

# # # ✝️ “Do religion or spirituality play a role in your life?”

This question surprises many people. Therapists may ask because spirituality or religion can:

* 🌟 provide comfort and meaning
* 🧭 influence values and coping
* 🤲 shape family dynamics
* 💔 contribute to shame, conflict, or trauma for some individuals

The answer may be:

* “very important”
* “not important at all”
* “complicated”

All of those answers are clinically relevant.

---

# # # 🔒 “Are there guns or weapons in the home?”

This question may be asked during certain psychiatric diagnostic evaluations, especially when there are concerns related to:

* ⚠️ suicidality
* 💥 impulsivity
* 🚩 domestic violence
* 📝 safety planning

The goal is safety assessment — not politics or judgment.

---

# # # 💊 “What medications are you taking?”

Even when someone is attending therapy instead of psychiatry, medications can affect:

* mood
* energy
* anxiety
* sleep
* concentration
* appetite

Therapists also want to know what treatments have or have not helped in the past.

---

# # # 💤 “How are you sleeping?”

Sleep problems are deeply connected to mental health. Poor sleep can worsen:

* 😰 anxiety
* 😔 depression
* 😡 irritability
* 🧠 concentration difficulties
* ⚡ emotional reactivity

Sometimes improving sleep becomes one of the first treatment goals.

---

# # # 💼 “How are things financially or at work?”

Mental health does not exist in isolation. Financial strain, burnout, unstable housing, caregiving stress, toxic work environments, and unemployment can deeply impact emotional wellbeing. 📉😮‍💨

Therapists often assess environmental stressors because symptoms are sometimes connected to chronic stress — not just internal emotional struggles.

---

# # # ❤️ “How are your relationships or romantic relationships going?”

This can feel especially uncomfortable during a first psychiatric diagnostic evaluation 😅 but relationship dynamics often affect:

* self-esteem
* stress levels
* communication patterns
* attachment
* depression and anxiety symptoms

Romantic relationships can significantly impact emotional wellbeing, coping, conflict patterns, and overall stress levels.

Clients can always decline to answer or revisit topics later.

---

# # 💙 A Reminder For New Clients

It is completely okay to feel nervous, guarded, emotional, or even skeptical during a first psychiatric diagnostic evaluation. Many people are discussing deeply personal topics with a stranger for the very first time. 🛋️✨

You are allowed to:

* ❓ ask why a question is being asked
* 🛑 say you are uncomfortable answering
* ⏳ revisit topics later
* 🌱 move at a pace that feels emotionally safe
* 🗣️ clarify misunderstandings
* 🤝 participate collaboratively in the treatment process

A psychiatric diagnostic evaluation is not meant to judge, criticize, or “pick apart” your life. The purpose is to better understand your experiences, symptoms, stressors, strengths, relationships, coping skills, and support systems so treatment can be individualized to your specific needs. 🧠💭

Many questions that initially feel random, uncomfortable, or unrelated are often part of understanding the larger clinical picture and helping therapists provide more appropriate care and support. ✨
