🏈 Need Your Child’s Sports Physical Form? ⚽🏀🏐🥁🪊

Accessing it is easy through the patient portal:
✅ Log in to the patient portal
✅ Select your child’s account
✅ Click Documents
✅ Open the most recent Sports Physical Form

Can't find a sports physical form? No problem! Simply send a message to your child’s provider through the patient portal and request that a form be completed. You will only need the last page, clearance form, to give to your child’s school.

Stay game-day ready and make sure your athlete has the paperwork they need for the season!

📚✏️ Back-to-School Reminder: Do You Need Vaccine Records for School? ✏️📚

As the new school year approaches, now is a great time to make sure your child is up to date on all required school vaccines. Missouri schools require certain immunizations for students entering Kindergarten, 8th Grade, and 12th Grade.

✅ Kindergarten • DTaP • Polio • MMR • Hepatitis B • Varicella (Chickenpox) • Hepatitis A (recommended)
✅ 8th Grade • Tdap • Meningococcal (MCV4)
✅ 12th Grade • Meningococcal (MCV4 booster after age 16) • Meningococcal B (recommended)

Need a copy of your child's vaccine record? It's easy to access through the Athena Patient Portal!

Log in to your Athena Patient Portal account.
Select your child.
Click My Health.
Choose Immunizations to view, download, or print your child's vaccine record.

⏰ If your child is due for vaccines, don't wait until the last minute! Call our office to schedule an appointment so they're ready for a healthy and successful school year.
