Our patients are at the center of all we do. We use a model called “Patient-Centered Medical Home” to guide our service. It means your primary care provider heads a team of nurses, health educators, pharmacists and others (like specialists or therapists) who work together to help YOU get healthy and stay healthy. You are the centerpiece of the team — the most important member! In practice, this me

— Get to know you: We will learn about you and your family and life situation. We will keep accurate records and update them every time we are in contact. We will listen to you because you are a full partner in your care.

— Communicate with you: Listen to your questions and concerns, and answer them all in a way you understand. We are happy to explain your health situation clearly and make sure you know all the options for care. We want to help you make the best decisions for your care.

— Support you: We will help you plan for your care, set goals and help you meet those goals. We offer information about classes, support groups and other resources to help you stay healthy. We refer you to other trusted experts when necessary. In this approach, we expect you to take charge of your health, be active in your own care, and talk with us about any changes, concerns or challenges. If you receive care from any other health providers, let us know so we can coordinate the best care possible. What a WIN! Swope Health is incredibly honored to be the recipient of Blue Cross and Blue Shield of Kansas City donation for August 2026!
Shut Out the Stigma is a partnership between Blue KC and the Kansas City Royals, recognizing behavioral health as a part of overall health and well-being, and removing the stigma around mental health support. As the August recipient, Swope Health KidsCARE will receive Blue KC's generous donation of $5,220, totaling the number of shutout innings that KC Royals pitchers recorded in August -- 174! -- at $30 per inning.
Tremendous thanks to Blue KC and The Kansas City Royals Foundation for supporting our work and investing in the health and well-being of Kansas City's youth. You're knocking it out of the park for our community!
