When therapy is excluded from a state's Medicaid PDPM model, some facilities assume the service just doesn't get provided. Medicare Part B says otherwise.

For the 63-77% of nursing home residents who are dual-eligible, Medicare Part B remains a viable reimbursement pathway for medically necessary therapy. But capturing it requires standardized assessments, objective documentation, and a clinical team that knows what to look for, including the gradual functional declines that don't announce themselves. Therapy evaluation coding is one of the most scrutinized areas in rehab documentation. Getting it wrong doesn't just create denials. It creates audit risk.

For physical and occupational therapists, selecting the right CPT evaluation code depends on the complexity of the clinical presentation, not habit or historical patterns. The 97161-97163 codes for PT and 97165-97167 for OT each carry specific documentation requirements. Currently, there is no reimbursement difference but Medicare is auditing to analyze trends. This may result in reimbursement adjustments for low, moderate, and high complexity evaluations in the future.

Understanding what supports each level, and documenting accordingly, is what separates compliant billing from recoverable overpayments.
