A federal judge has declined to temporarily block the Trump administration’s new Medicaid work requirements—but this legal fight is far from over.
Under the policy, some adults covered through Medicaid expansion may need to complete at least **80 hours per month** of work, education, volunteering, or other approved community activities to keep their coverage.
For chronically ill patients, this raises a serious question:
**Should patients have to prove they are too sick to work to keep their health insurance?**
In some cases, patients may need a physician’s note or additional medical documentation showing that their condition prevents them from meeting the monthly requirement.
Healthcare organizations are concerned that this could:
• Add more paperwork for physicians and clinical teams
• Put doctors in the position of judging whether a patient can work
• Take time away from direct patient care
• Put vulnerable patients at risk of losing coverage because of documentation problems
It is important to understand that the judge did **not** rule that the policy is legally valid.
The court only decided that the states had not yet shown enough immediate and irreparable harm to justify temporarily blocking the rule.
The injunction was denied **without prejudice**, so states may ask the court again if new evidence of harm emerges. The broader lawsuit will continue as states prepare for the **January 1, 2027 implementation deadline**.
This case shows how healthcare coverage can depend on more than medical need. Documentation, administrative systems, deadlines, and access to physicians may all affect whether a patient remains insured.
Healthcare providers and Medicaid organizations should start preparing for more patient questions, exemption requests, eligibility reviews, and administrative work.
