THIS IS WHY YOUR SHOULDER STILL HURTS

Most people go right into doing rotator cuff/localized shoulder work even though the shoulder joint is dependent on the ribs, cervical spine, scapula, and clavicle to function as best as it can.

You can do those strengthening exercises, but they would probably be much more effective if you address those other things first, or at least in tandem with the shoulder stuff.

I didn't make the rules. Anatomy and Biomechanics did. And a lot of the time if you improve those things, the shoulder gets better way quicker.

So feel free to do your ER/IR drills, BUT make sure your scaps can move and that you can rotate your torso.

Stop chasing your pain and get to the root of your issue.

These are the 3 steps I follow every time I work with someone to understand the factors contributing to their pain, create a plan to change those factors using manual therapy and movement, and integrate those changes through strength.

Where a lot of approaches fail is that they're treating the body like a machine rather than a whole integrated unit. Just because your knee is hurting that doesn't mean you need to do things at the knee. Pre-set protocols that an insurance company deems best care don't take into account of the history and circumstances of the individual, especially as you have more chronic issues.

The typical model doesn't allow for a thorough assessment process because there's an assumed direction based on the code that's entered into the system, because you're seeing someone who is shoehorned into a "flow" based on where they work, or they literally just don't have the time because they need to focus on quantity of people treated rather than quality.

It's a consequence of the overarching system, not the individual practitioner.

This process shown here has proven time and time again to be successful. It's individualistic, combines different modalities based on needs, and works to create long lasting change through strength rather than stopping the second you feel better.

If you watch people exercise, hike, and just generally live their life with no restriction and think "I wish I could do that, but my pain won't let me", then you NEED to address it ASAP.

The only reason you should avoid an activity is because you don't have interest in doing it. The second you start to limit what you do because of a pain or injury is the second you should consider seeking help.

Because I've got news for you... avoiding it and hoping it goes away doesn't usually work. Life is meant to be lived, not lost. Being afraid to move and do things will actually make your pain and injury worse in the long run (I actually have a 🎙️ podcast🎙️ episode where I talk about this a bunch - you can listen via the link in my bio).

The great thing is that you can do something to decrease your pain, increase resilience and strength, and increase functionality... but you need to be an active participant. Every day I do this with people and have seen people completely take their life back from pain. You can do it to, you just need to say "yes" and make the change.

Her back pain was taking too much from her.

Sleep.
Comfort.
Ability to enjoy work.
Enjoyment of life and training.

What makes it worse is that she's tried the things. Chiro, PT, Massage, different forms of medication, and none of it did much. They all felt like bandaids for an issue that needed a long term solution rather than a quick fix.

When she came in for her eval we immediately found stuff that seemed to be related to her pain, the biggest thing being a lumbar joint compression strategy that wouldn't let go, causing a bunch of issues with her movement and core function.

After a few sessions of working through that issue, coaching breathing and bracing drills, giving her strategies at home to really make meaningful change, she is feeling better, less pain, more confident that she can work and not be debilitated, and is excited to workout again. If there is a flare up from a crazy day at work, she has a staregy that works and that makes her more resilient in the long run.

THIS IS FOR ALL THE MOMS OUT THERE

Your C-Section scar shouldn't be ignored. PERIOD. C-sections are an incredibly intense surgery for the mother. They are physically traumatic and can often emotionally be traumatic, as they're not always planned and can be a stressful experience in general.

When you get a C-section, they have to cut through 7 layers of tissue. 4 of those layers are parts of your re**us sheath, which are the tendons of important musculature that also house your re**us Abdominus (6 pack muscle). This whole region is integral to spinal stability, rib movement, pelvic control, and global force transfer, so it's kind of important...

Not to mention that there are tons of lymphatics, nerves, and blood vessels in the area that can get disrupted from the surgery and scar tissue formation afterwards.

I see a lot of women who have pain and dysfunction caused by their C-section scar, even 10+ years after the C-section occurs.

Because scars cause fascial distortions, disrupt the brains mapping and proprioception of the region,and interfere with muscle contraction, they can be facilitators that cause both local and global inhibitions of movement patterns.

Never ignore your C-Section scars (or any for that matter). They are often related to pain and movement dysfunction and can be a key to your healing.

PT couldn't touch it.

Labral repair. Sciatica. L5-S1 herniations. A whole leg full of pain that had been there for over a year.

4 sessions later: No pain.

Here is what we found when we actually looked.

Her right lumbar erectors were overworking and compressing her spine and inhibited her left multifidus. Her intrinsic core had checked out entirely, so her nervous system built a compensation pattern just to feel stable. And MRS testing pointed to radiculopathy at L5 and S1 that orthopedic testing confirmed.

A complicated picture. But once you find the root, the path forward is clear.

We released the overworking erectors, reactivated the multifidus, added breathing and bracing work to lock the change in. Used nerve flossing to keep the nerve happy. Then layered in strength and coordination work for the entire left leg because downregulated nerve flow creates weakness and atrophy that has to be rebuilt.

We had so much progress that we aren't even working on that anymore.

This is not a lucky case. This is a normal day at Uplift.

Have you been in this position before? When your pain and injury becomes so limiting that you question doing even the most basic tasks?

3 things I would do if I had back pain

1) Integrate breathing and bracing strategies. your diaphragm is one of your most important core muscles, and is integral to other core muscles.
2) Load and move my spine through ranges that are tolerable. Stiff and stagnant spine is a sad one. Let that puppy move if you can
3) Integrate with hinges and squats. These motions require spine stiffness, load the spine, and integrate other joints with some decent load. Presses and pulls can work too, but I find these other movements are more directly related.

Nobody should ever lose their life to chronic pain.

Period.

If you are, I'm sorry. But don't give up. Don't rely on passive quick fix/bandaid approaches.

You need assessment. You need specificity. You need individualization. And you need to be active in it.

I've always said "you can't out massage poor motor patterns". You need to have a movement component in some way shape or form, and one that is tailored towards your goals and assessment.
