Here is a very gratifying implant revision case!

This is a petite 42 year old female who had a history of implant placement above the muscle many years ago, out of state. She was bothered by an unnatural breast shape and discomfort. With her goals in mind, the following procedure was designed:

- Implant exchange (from 375 cc, textured, shaped, moderate profile devices to 240 cc, smooth, round, high profile devices)
- Conversion to a subpectoral pocket
- Partial capsulectomies
- Inverted-T mastopexy

The implant selection and “pocket conversion” was recommended to better match her chest wall and provide a more natural upper pole, especially after distortion of the previous implant pocket above the muscle.

She is much more comfortable and confident with her appearance. Shape and thoughtful implant placement beat volume every time.

This is so fun to see! Here is a 51 year old female who had a history of saline breast augmentation - she was bothered by significant rippling and an unnatural breast shape.

We deflated the saline implants under local anesthesia in the office first (middle image), and this helped her decide if she wanted to have a lift along with removal of the implant shells, or if she wanted to have another implant placed.

The patient ultimately decided to have the implants removed and the breast tissue reshaped with a mastopexy, using inverted-T incisions. She is so shown 3 months from surgery and is much happier!

🏆 This is an impressively fit 51 year old female who wanted a tasteful improvement in breast volume. To accomplish her goals, we decided on 245 cc Motiva implants (round, semi) placed in the subfascial plane, meaning that the implant stays above the muscle - a particularly good choice for patients that are serious about the gym.

⚖️ This is a 53 year old female who was bothered by an unnatural breast shape after previous augmentation. Notice the higher implant on the right side from capsular contracture, tethering of the upper-outer quadrant on the left, and descent of the overlying breast tissue.

She is 3 months from implant exchange (staying above the muscle), capsulotomies (scoring/releasing the pocket), and a circumareolar mastopexy. We used Motiva implants (ergo design, demi profile, 230 and 245 cc), which give her a much more natural shape than her previous implants.
