“The guidelines say…”

Okay… which guidelines?

As an auditor, “that’s how we’ve always done it,” “that’s what I was taught,” and “everyone codes it that way” aren’t the same thing as published guidance.

When you make an audit finding, you should be able to answer one very important question: Where is it written?

Can you point to the regulation, manual, official guidance, payer policy, or another authoritative source that supports your conclusion?

Because there can be a BIG difference between what the guidance actually says and what has simply become common practice over the years.

It’s more than getting to the right conclusion. When you put your name on an audit, you’re standing behind the entire process—the sample, methodology, guidance, documentation, rationale, and consistency of your findings.

A strong audit should create a clear path from the medical record to the applicable guidance to the final conclusion. Another experienced auditor should be able to review your work and understand exactly how you got there, even if they ultimately interpret the issue differently.

Before you finalize your next audit, ask yourself: Would I be comfortable putting my name on this if another experienced auditor reviewed every step?

If the answer is yes, you have more than an opinion. You have a defensible audit.

You can have all the right documentation in the chart… and still have an audit problem.

In substance use disorder services, it’s not enough for the assessment, treatment plan, services, and progress notes to simply exist. They need to connect and tell one consistent patient story.

Does the assessment support the treatment plan? Do the services align with that plan? Does the documentation show how the patient is progressing?

Because there’s a big difference between “it’s in the chart” and “the chart supports the care.”
