The question isn’t “Which weight-loss medication is best?”

That’s the question social media asks.

The question medicine asks is:

Which medication has the strongest evidence for the diseases this patient actually has?

We’re no longer treating weight in isolation.

We’re treating cardiovascular disease, chronic kidney disease, type 2 diabetes, sleep apnea, fatty liver disease, and obesity—all at the same time.

The future of medicine isn’t about chasing the biggest weight-loss number.

It’s about matching the right therapy to the right patient at the right time.

That’s where the science is headed.

And honestly, it’s about time.

Monday Leg Day 💪🏻

Today’s workout:

• Leg Extensions
• Leg Press
• Abductors
• Adductors
• Hamstring Curls
• Calf Raises
• Cable Crunches (just for the hell of it 😏)

For each exercise, I perform 3–4 sets of 8–10 repetitions, emphasizing a slow eccentric phase and training close to muscular fatigue.

Glutes are on Wednesday. All the squats, hip thrusts, and kickbacks are next. Stay tuned.

Someone asked me, what has been the biggest sign of progress for you mentally? The biggest mental shift has been understanding that self-worth and body image are not the same thing. I’m still a work in progress, but I no longer let every perceived flaw define how I see myself.

The hardest opponent I’ve ever faced wasn’t in the gym. It was the version of me that thought I had to earn my worth.

More reps. More weight. More success. More validation. But growth starts when you realize you were enough long before you became stronger.

This is your reminder to keep showing up anyway 💙.

The science has moved forward.

Now access needs to catch up.

Obesity is a chronic disease, yet millions of patients still face barriers to evidence-based care because of cost, insurance restrictions, limited access to specialists, and weight stigma.

We can do better.

Weight loss is the mechanism.

Health improvement is the goal.

I was in the room at ADA 2026 when the TRIUMPH-1 retatrutide data were presented.

The room was packed. The energy was electric.

As obesity specialists, we’re always looking for therapies that don’t just reduce body weight, but improve the diseases that come with obesity.

Weight loss is the mechanism.

Health improvement is the goal.

And while the science is exciting, it’s important to remember that retatrutide remains an investigational medication and is not commercially available.

So if someone offers to sell you “Reta,” ask yourself one question:

What exactly is in that vial?

Menopause and weight gain are real—and we don’t talk about it nearly enough. These findings, based on post-hoc analyses of more than 1,500 female participants in the ATTAIN-1 and ATTAIN-2 clinical trials, were presented at the American Diabetes Association (ADA) 86th Scientific Sessions.

This oral GLP-1 therapy showed meaningful weight loss in women across all stages of menopause: before, during, and after the transition. Importantly, these benefits were also seen in women living with type 2 diabetes—a population in whom weight loss can be especially challenging.

As an obesity medicine physician, what excites me isn’t simply the number on the scale. It’s the potential for meaningful improvements in metabolic health, quality of life, and long-term outcomes.

Menopause is one of the most hormonally complex periods of a woman’s life. Women deserve evidence-based treatments that acknowledge those challenges and help them succeed.

The science continues to move forward—and that’s good news for millions of women.
