Every TRU PACE participant has a story. A favorite memory. A special tradition. A goal for tomorrow. A person they love. A place they call home.

This National PACE Month, we're proud to celebrate the lives touched through comprehensive, compassionate care.

Your story matters. Your voice matters. You matter. Thank you for being part of the story.

What you eat can support how you feel, think, and move through the day.

For older adults, nutritious meals can play an important role in whole-person well-being. Foods that support brain health often include colorful fruits and vegetables, leafy greens, whole grains, beans, nuts, fish, and healthy fats.

These foods can help fuel the body while supporting energy, daily routines, and overall wellness. A brain-supportive plate might include:

1. Leafy greens like spinach or kale
2. Berries or other colorful fruits
3. Whole grains like oatmeal or brown rice
4. Beans, lentils, nuts, or seeds
5. Fish or other protein-rich foods
6. Healthy fats like olive oil or avocado

TRU PACE supports whole-person care by helping eligible older adults access coordinated care, supportive services, meals, and personalized care planning designed around their health, routines, and daily life. Find out how: trupace.org

This September, we're celebrating the partnerships that help older adults stay healthy, connected, and independent. Today, we're proud to spotlight Meals on Wheels of Boulder!

Every day, our partnership makes a difference for the people we serve. Whether participants receive meals at the TRU PACE Day Center or enjoy home-delivered meals, TRU PACE and Meals on Wheels of Boulder work together to provide support that helps older adults continue living independently and with a higher quality of life.

Kim and Lehshel from Meals on Wheels of Boulder hear incredible stories from participants about the impact of these services. Clients share how meals and supportive connections have improved their health, brought them joy, increased their comfort, and helped them feel safer and more cared for.

One participant, Susan, has received home-delivered meals for almost 10 years. She often says the meals have "saved her life" and doesn't know what she would do without them. Today, thanks to the partnership between Meals on Wheels of Boulder and TRU PACE, she receives even more support to help her remain independent and thriving.

Thank you, Meals on Wheels of Boulder, for helping us nourish lives, build connections, and support older adults throughout our community.

Therapy can support the life someone wants to keep living.

At TRU PACE, physical, occupational, and speech therapy are part of the all-inclusive care model and may be recommended to help participants stay active, confident, and supported in daily life.

Physical therapy can support strength, balance, walking, and steadiness.

Occupational therapy can support daily tasks like dressing, bathing, cooking, and moving safely around the home.

Speech therapy can support communication, safe swallowing, memory strategies, and ways to make talking and understanding easier.

Each therapy plan is personalized and added to a participant’s care plan when the participant and care team agree it can support their health and independence.

With TRU PACE, therapy is connected to the full care team, helping participants receive support that reflects their goals, their abilities, and the home life they want to maintain.

A little preparation can bring a lot of peace of mind.

When you are aging, or caring for an aging loved one, having key information in one easy-to-find folder can help the whole family feel more prepared. It can also make it easier to share accurate details during appointments, transitions, or urgent situations.

2. Doctor and care provider contacts
3. Emergency contacts
4. Insurance information
5. Known allergies and medical conditions
6. Copies of important care documents

Understanding TRU PACE starts with a conversation.

Before applying, an enrollment specialist can help older adults and families understand what TRU PACE is, how the program works, and what eligibility may look like.

TRU PACE provides care for older adults who want to continue living safely in their community while maintaining independence. The program can support participants at home and in the community through a managed medical plan designed to help meet their healthcare needs.

An enrollment specialist can help explain:

1. Whether someone meets the basic age requirement of 55 or older
2. Whether they live in the TRU PACE service area
3. How nursing facility level of care is determined by the state
4. What it means to live safely in the community with TRU PACE support
5. How care works through TRU PACE contracted providers

Taking the first step can feel more manageable when you know what questions to ask.

A strong caregiving plan gives everyone a clear place to start.

Caregiving often includes many moving parts: appointments, medications, transportation, meals, home routines, social connection, and changing health needs. When responsibilities are shared, a simple plan can help families stay organized and confident.

Helpful steps include:

1. Write down the most important care needs
2. Match each task with a specific person
3. Create a shared calendar or checklist
4. Keep notes from appointments in one place
5. Make time to talk through updates together

Eligible older adults at TRU PACE receive coordinated support through an interdisciplinary care team and a personalized plan of care designed around their needs and goals.

A steady routine can make the day feel more manageable.

For older adults, daily routines can bring comfort, confidence, and a stronger sense of stability. Familiar rhythms like regular meals, movement, medication reminders, social time, rest, and meaningful activities can help support physical, emotional, and social well-being.

A daily routine may help you:

1. Feel more grounded throughout the day
2. Keep meals, medications, and appointments on track
3. Build in time for movement and rest
4. Stay connected through regular social moments
5. Create a sense of purpose and independence

TRU PACE supports whole-person care by helping eligible older adults build support around their health, routines, goals, and daily life.
