DR. HAZLEY EARNED HIS DOCTOR OF MEDICINE degree from Ross University School of Medicine and completed his Family Medicine residency at Meharry Medical College in Nashville, where he served as Chief Resident. He has spent his
career caring for patients throughout Middle Tennessee and is passionate about building lasting relationships that support lifelong health and wellness. BOARD-CERTIFIED IN FAMILY MEDICINE, Dr. Hazley provides preventive care,

🩺Brande’s last day🩺

She received an outpouring of gifts, cards, and well wishes. Thank you to everyone for your kindness and understanding.

Brande wanted to make sure all of her patients received a letter from her explaining her leaving. We tried to make sure everyone received a letter in the mail, an email, or both. If we missed you, we apologize and a copy of the letter is attached.

The Primary Healthcare Group will forever be grateful for the impact Brande made in our lives and our community. She will be greatly missed, but we are happy for her and wish her the best in her future endeavors.❤️
