Bladder irritant lists are not adequate treatment.

If you’ve been given a list of bladder irritant to help treat your urinary urgency, you need to expect more of your provider.

They can be helpful in general to help you to see if you are consuming an excess amount of these items, but at the end of the day, you should be able to eat and drink whatever you want and not have your bladder go haywire.

If your pelvic PT stops at just giving you this list, we need to find another pelvic PT.

Your pelvic PT should be working on helping your system to get to the root cause of why your bladder is reacting the way it is and helping you to build resilience in your bladder and nervous system so that you can tolerate more. Not consume less. 💁🏻‍♀️

This goes for things like pelvic floor PT. And also goes for things like imaging and lab tests.

Physical therapy codes are physical therapy codes.
➡️ manual therapy ➡️ neuromuscular reeducation ➡️therapeutic activities ➡️ therapuetic exercises

There is no special distinction for pelvic floor physical therapy versus physical therapy for your knee your shoulder, your hip, your back.

Sometimes a provider might choose to become an out of network provider, but it doesn’t change the fact that physical therapy codes are physical therapy codes. That only changes who gets reimbursed.

IC/BPS is a diagnosis of exclusion. And what that means is that the provider that is diagnosing, needs to rule out everything else it could possibly be.

Is IC/BPS a “real” diagnosis? OF COURSE, YES!

But is it chronically underinvestigated? YES! 1000% YES.

The three most common things that I find in people with “IC/BPS” that are missed (outside of PFD and central sensitization) are:
1️⃣ GSM or GSL
2️⃣ endometriosis or adenymyosis and
3️⃣ recurrent (true, culture positive) UTIs

If you feel like your provider, just slapped the IC diagnosis on you without fully investigating, then this is your permission to find another provider.

(And in the meantime, no matter what the diagnosis, pelvic floor PT can significantly help!!) 💁🏻‍♀️

If you have started pelvic floor physical therapy and then stopped because it’s been “too painful”, this post is for you.

Firstly, I am so sorry that that has been your experience! 😢

Secondly, I want you to know that pelvic floor physical therapy should not hurt. Your pelvic floor physical therapist should be willing and able to make adjustments to your treatment so that it is the most comfortable for you.

It is true that sometimes we run into situations where something might be very sensitive, painful in the moment and/or reactive to an intervention. But you should always have an open line of communication with your pelvic floor PT during the sessions and good communication on what to expect after.

If you come back and report significant pain, your pelvic floor PT should be willing and able to make adjustments. If they can’t or won’t please know that pelvic floor physical therapy didn’t fail … but rather your pelvic floor physical therapist failed you. 🩷 And try again with another provider if you can.

Do you have a small bladder?!

The short answer is that having a bladder where the physical bladder capacity is physically small is actually quite rare! So the likely answer is no. 🫣

Things that can make you FEEL like you have a small bladder and create symptoms of urinary urgency and frequency are:
OAB
IC/BPS
Enlarged prostate
UTI
Pelvic floor dysfunction
Diabetes
Bladder irritant intake

The good news is, there is treatment for symptoms of urinary urgency and frequency and bladder pain!

Pelvic floor physical therapy is a great treatment option for most if not all of the symptoms and conditioms listed above. We can also refer you to the appropriate physician if needed! 💁🏻‍♀️ But stop blaming your “small bladder” and actually get the help to stop the symptoms! 😉

▶️ Normal urinary frequency = 4-7x/day (daytime) and 0-1x/night (nighttime - and its mostly 0! exceptions are for pregnancy and over the age of 65)

This translates to being able to hold from 2 to 4 hours - without a problem! No urgency, no anxiety no rushing to the bathroom, no leakage.

And this is regardless of how much water you’re drinking. (because I know all y’all just did a “yeah but” I drink a lot of water 🤨)

And no, you likely do not have a small bladder either. 🫣

There are three recognized phenotypes of IC/BPS:
1️⃣Hunner’s lesions (less than 10% of all cases and less than 4% if under 50)
2️⃣Pelvic Floor Dysfunction
3️⃣Central Sensitization (Nervous system dysregulation)

No matter your type, pelvic PT can help. If you have Hunner’s lesions type, your medical management needs to be different. And for ALL types, you need to be worked up for possible endometriosis and/or hormonal fluctuations that can be contributing to or causing symptoms.

First stage labor positions || Supported side lunges with pelvic tilts
~
Loved how this felt on my entire lower body during my first stage of labor! I played with my foot position to see how that felt at my hips and pelvis. I chose a neutral foot, but toes out wide felt good too.

In this video, I started out rocking/lunging gently side to side with a ball to support. Then, keeping my legs wide, I paused ti do some pelvic tilts. Keeping my forearms on the ball increased the stretch into my lats.

Important parts here are:
✅ keeping the movement flowing
✅ not overstretching if something didn’t feel good
✅ rocking forward and backward (pelvic tilting) helps to open the pelvis

Could have also:
▶️ taken away the ball and just used the bed
▶️ continued this when baby was moving into midpelvis

This is one of MANY first stage labor postions that can help! Best to work with a pelvic floor Pat who can assess the ability for your body to get into these positions without straining AND a doula can help suggest modifications for the best outcomes!
