❗️SPORTS PHYSICALS❗️
Reminder for all Union County students and parents, we are doing free annual sports physicals Thursday, June 4th at Union Direct Primary Care, Lake Butler.
Please have ALL of the paperwork filled out & signed by a parent/guardian prior to arriving. If your paperwork is not filled out and signed we will not be able to complete the physical.
If you are an incoming 9th grader or haven’t played a high school sport before, please expect to get an EKG done by Who We Play For for $20. Use the QR code on their flyer in comments to register with them and schedule your appointment.

We have some exciting news!!!
Brooke Waters, APRN, FNP-C is a certified Family Nurse Practitioner. She has been in the healthcare field for 5 years. She received her Bachelor of Science in Nursing from the University of North Florida in 2021 and her Master of Science in Nursing from the University of West Florida in 2025.
You may have already had the pleasure of meeting Brooke, who worked as a nurse in our office while she was completing her Master’s degree!
Brooke is a lifelong resident of Union County and graduated from Union County High School. She enjoys spending time with family and friends and traveling. She looks forward to giving back to her community, providing affordable and convenient healthcare.
