On June 6th 2025 a LDH survey found we were deficient in the following areas.

1) our administrator had not yet had a yearly review, that was corrected on 8/18/25

2) the agency had a lapsed agreement with a contract provider. This was corrected 6/6/2025

3)the agency was found to have given a wrong hire date for a worker, this was corrected and clarified. no services were provided by this worker until after background, orientation and training were provided to that worker.

4) the agency was found to have completed all necessary background, medical screens and training for all workers prior to provision of services, however it was found that the 3 letters of recommendation were not present , Agency is working on correcting this issue currently with a deadline of 12/15/2025.

5) Despite the agency training workers on infection control and universal precautions and following all required safety protocols in facilities it did not have a Infection control plan that met all LDH requirements. That has been corrected and is now in place as of 7/7/2025.

6) Acadian Holistic's emergency response plan was updated with a safe zone that was displayed on the office wall and in the subsequent hall way to ensure protection of individuals in the office .completed 6/10/25

7) Acadian Holistic was found to not have reviewed the Emergency preparedness plan this year and updated it as necessary. This was completed on 8/18/25 as it was reviewed and adopted by the governance board.
