People always ask… “Is it a fracture or a break?”
Truth is — they’re the exact same thing. A fracture = a broken bone. 🦴

As orthopedic surgeons, we just use more specific terminology to describe the injury because not all “breaks” are created equal.

We describe:
• Displacement – did the bone move out of place?
• Angulation – is the bone crooked?
• Rotation – is the bone twisted?
• Location – is it diaphyseal (shaft) or metaphyseal (near the end of the bone)?
• Open vs closed – did the bone break through the skin or not?

Those details matter because they determine stability, treatment, surgery needs, healing potential, and long-term function.

So yes… a break is a fracture.
But every fracture tells a different story. 🔩🩻

Open ankle fractures are not just “broken bones”—they are complex, high-energy injuries with serious soft tissue implications.

When the ankle is exposed through the skin, the challenges go far beyond the fracture itself:
• Soft tissue damage: skin, tendons, and ligaments are often severely compromised
• Fracture blisters & wound issues: a sign of significant swelling and skin injury that can delay or complicate surgery
• Infection risk: one of the most critical early concerns
• Swelling & vascular compromise: can make timing of surgery extremely delicate
• Cartilage injury: often underestimated at presentation

Even after successful fixation, the journey isn’t over:
• Post-traumatic arthritis is common due to cartilage damage at the time of injury
• Stiffness, chronic pain, and swelling can persist long term
• Recovery is often measured in months—not weeks

These injuries require meticulous timing, careful soft tissue management, and often staged treatment to optimize outcomes.

In orthopedics, the fracture is only part of the story—the soft tissue envelope often determines everything! With Pt permission!
