Our adopted desert tortoise, Wild Bill, is enjoying some gentle head and neck rubs—and reminding us how meaningful moments of care can be. 🌿

At Skye Canyon Post Acute, recovery is personal. Physical, occupational, and speech therapy help patients work toward strength, everyday skills, and greater independence. 💚 What does short-term rehab look like?

At Skye Canyon Post Acute, recovery means having support for both the big goals and the everyday moments.

🩺 Skilled nursing support to help manage care needs throughout your stay.

💪 Personalized rehabilitation with physical, occupational, and speech therapy based on each patient’s needs, with therapy five days a week.

🍽️ Nourishing meals with attention to individual dietary needs.

🎨 Meaningful activities that offer opportunities for connection, enjoyment, and encouragement.

🏡 Thoughtful discharge planning with our case manager and social worker. Our case manager helps coordinate ongoing care, equipment, home health services, and follow-up needs. Our social worker provides emotional support, connects patients and families with community resources, and helps address concerns or barriers to returning home. Together with nursing, therapy, patients, and families, they help prepare for a safe, supported transition to the next step.

With private suites and a caring team, we help patients work toward greater strength, confidence, and independence—one step at a time.

🍂💚 Happy National Case Management Week!

At Skye Canyon Post Acute, we’re celebrating Anabelle Co, LPN, Case Manager, and the compassion, dedication, and careful planning she brings to our patients’ recovery.

As rehabilitation helps patients rebuild strength and independence, case management helps prepare for what comes next. Working alongside nursing, therapy, physicians, social services, patients, and families, case managers help:

🏡 Plan for a safe transition home based on each patient’s needs.
🤝 Keep patients, families, and the care team connected.
🩺 Coordinate needed follow-up care and home health services.
🦽 Help arrange necessary equipment and community resources.
💬 Identify barriers and help families understand the next steps.

These details matter. Having the right support, equipment, and care arrangements in place helps patients continue their recovery more safely after discharge.

🏆 Five Stars Golden Trophy Recipient

Skye Canyon Post Acute is proud to celebrate our CMS 5-Star Overall Rating and the gold trophy designation on Medicare’s Care Compare! This is a significant accomplishment for any skilled nursing facility—and a proud moment for our leadership and entire team.

What does the golden trophy mean?
CMS displays this symbol to identify higher-performing facilities that meet its qualifications for the Risk-Based Survey program. Eligibility includes a five-star overall rating, at least a three-star staffing rating, and additional requirements involving inspection history, safety, and data accuracy. The trophy recognizes qualifications beyond the overall five-star rating.

What does it take to get here?
Consistent effort, accountability, and teamwork. It takes careful clinical assessments, individualized care plans, accurate documentation, infection prevention, attention to safety, ongoing staff education, and a willingness to keep improving.

It also takes the everyday work that patients and families experience firsthand: a nurse noticing a change, a therapist encouraging another step, a nourishing meal prepared with care, a clean and welcoming room, and a team member taking time to listen.

Did you know desert tortoises spend the colder months in brumation, their version of hibernation? As temperatures cool, their activity, appetite, and metabolism gradually slow down for their winter rest. 🍂💤

For now, Bill is enjoying his greens and reminding us to savor the little things—one happy bite at a time! 💚
