CMS finalized the Medicaid work requirements rule on June 1, and it came in stricter than states expected. For agents, this is a wave of people who will lose Medicaid and need new coverage, starting this summer in some states.

Here is what you need to know.

The requirement: Starting January 1, 2027, adults 19 to 64 in expansion states must complete 80 hours a month of work, school, job training, or community service to keep Medicaid. It applies to all 41 expansion states. CMS projects 2.3 million people lose coverage in the first year.

The surprise: The medical frailty exemption got narrower. Having a qualifying condition is no longer enough on its own. The condition also has to significantly impair the person's ability to meet the 80-hour requirement. Fewer people will qualify for the exemption than states planned for.

The timeline: Most states start January 1, 2027.
