1 of 10 PEAK mentor homes in Kansas. We are 1 of 7 PEAK mentor home in the state of Kansas.

Only stand-alone Level 1 Age-Friendly Health System Participant in Kansas. Our elders live in one of three small healthcare neighborhoods within our home. Consistent teams of caregivers work to support the daily preferences and routines of elders living in each neighborhood. Building relationships, community and meaningful life are at the heart of our mission.
