Need physical therapy?
What does your first visit Intel?
What do you need to bring with you to the initial evaluation?
-First you may need a referral from your doctor
Every plan is different, you can contact your provider for that information or we can help assist you.
-After we verify insurance & payments our office staff will contact you to set up an Initial Evaluation
-For this evaluation please make sure to bring your drivers license, insurance cards also make sure to wear loose fitting clothes so that our therapist may get to your injury for easy treatment
-Show up to your appointment time 10-15 minutes early to fill out some paperwork -OR- you can print off the forms from our website, fill them out and bring them with you.

Select the "For Patients" tab then "Intake Form" Tab
-You will have a 1 hour examination , in this time our Therapist will
1) Discuss your condition
2) Develop a treatment plan
3) Assign home exercises
