They are navigating hormonal transition while carrying invisible levels of physical, emotional, cognitive, and caregiving responsibility.

This Mother’s Day, we want to acknowledge something that is often overlooked in women’s health:

Many women enter their late 30s and 40s already depleted long before perimenopause even begins.

They are:
• raising children
• managing careers
• supporting households
• caring for aging parents
• carrying emotional labor
• functioning through chronic stress and sleep deprivation

MAY IS MENTAL HEALTH AWARENESS MONTH.

But there is a critical gap in how we talk about women’s mental health in midlife.



Many women in their late 30s, 40s, and early 50s experience:

• increased anxiety
• low mood
• irritability
• emotional volatility
• reduced stress tolerance
• loss of motivation



And it is often labeled as:

“stress”
“burnout”
“just life”



Clinically, that is incomplete.



Because during perimenopause and menopause, the brain itself is undergoing neuroendocrine change.



Estrogen is not only a reproductive hormone.

It plays a central role in brain function.

It modulates:

• serotonin → mood stability and emotional regulation
• dopamine → motivation, reward, and drive
• GABA → calming and anxiety control
• cortisol signaling → stress response



During perimenopause, estrogen does not decline in a linear way.

It fluctuates.

And these fluctuations create instability across these systems.

Women in their 40s and 50s often present with symptoms such as sleep disruption, fatigue, changes in body composition, mood shifts, irregular cycles, or vasomotor symptoms. These changes are frequently related to the physiologic transition of perimenopause.

Perimenopause is not a single event, but a complex neuroendocrine transition involving fluctuations in ovarian hormone production and downstream effects on metabolic regulation, sleep physiology, thermoregulation, and cardiovascular health.

Thoughtful midlife care requires looking beyond a single symptom or laboratory value. It involves understanding how hormonal changes interact with metabolism, muscle physiology, sleep regulation, and overall health.

For some women, hormone therapy may be an appropriate and effective intervention. For others, lifestyle strategies, metabolic support, strength training, and sleep optimization play equally important roles.

Effective care requires looking at the whole physiologic picture.
