⚠️ SCAM ALERT – PLEASE SHARE ⚠️
We have been made aware that individuals are spoofing our phone number and contacting community members while pretending to represent our office.
These callers are asking for Medicare numbers and other personal information. Please be aware that LFPC should not call you unexpectedly and ask for your Medicare number, Social Security number, banking information, or other sensitive personal information over the phone.
The only calls you should expect from LFPC are from our office or clinical staff regarding your care, appointments, billing, or from a member of your Chronic Care Management (CCM) team if you are enrolled in that program.
If you receive a suspicious call that appears to come from our number:
✅ Do not provide any personal information.
✅ Hang up immediately.
✅ Call our office directly to verify whether the call was legitimate.
Unfortunately, scammers can make it appear as though they are calling from a trusted local phone number. We encourage everyone to stay vigilant and help spread the word to family members and friends, especially seniors who may be targeted by these scams.
