Our nurses help keep care close to home.
Across 51 communities in Coös and Grafton counties, NCHHHA nurses care for people in one of the most personal places possible: their homes.
Home-based nursing is unique because the care is not happening in a hospital room or clinic. It happens at kitchen tables, beside favorite chairs, in living rooms filled with family photos, and in the communities our nurses know well. Many times, nurses are caring for neighbors, familiar faces, and families whose stories are woven into the North Country.
This work takes clinical skill, independence, flexibility, and heart. Each day brings meaningful variety, the chance to build real relationships, and the opportunity to help patients and families feel supported where they feel most comfortable.
