Another successful year of our Sports Physical Clinic sponsored by Livingston Regional Hospital!

Special thanks to Dr. Mark Langenberg and Kelli Carr, FNP, of The Livingston Clinic and Josh Cooper, FNP of Livingston Family Medicine for doing sports physicals last night!

Because of the teamwork from The Livingston Clinic, Livingston Family Medicine, Livingston Surgical Clinic, and Roe Rehab, we were able to provide sports physicals for 179 athletes in a little over three hours!

Don’t forget if you missed this event, we will be at Clay County Family Wellness Thursday, April 23rd from 5PM - 8PM! See their page for more information!

The parent and student sections must be completed and signed before arriving.
Cash only – $10 per student.
Once you leave the clinic, you are responsible for your physical form.

Forms are the responsibility of the patient/parent. This office does NOT retain copies of completed forms. Replacement forms require a new appointment and additional fee. Don’t forget to turn the form in to your school or coach.

⚠️ This clinic is NOT appropriate for students with heart disease, current symptoms such as chest pain or shortness of breath, or other chronic medical conditions. Please see your primary care provider for a full wellness exam.
