We’re excited to offer two Sports Physical Clinics this year:
🏥 The Livingston Clinic
🗓 Thursday, April 16
⏰ 5:30 PM – 8:30 PM

🏥 Clay County Family Wellness
🗓 Thursday, April 23
⏰ 5:00 PM – 8:00 PM

✅ No appointment needed – first come, first served!

Important Notes:
The parent and student sections must be completed and signed before arriving.
Cash only – $10 per student.
Once you leave the clinic, you are responsible for your physical form. Don’t forget to turn it in to your school or coach.

⚠️ This clinic is NOT appropriate for students with heart disease, current symptoms such as chest pain or shortness of breath, or other chronic medical conditions. Please see your primary care provider for a full wellness exam.
