Not only do Home Care Connect's in-home services contribute to the patient's quality of life but it also helps prevent costly and inappropriate hospital stays. Initial evaluation and assessments to begin services and case management are the responsibility of Registered Nurses. The supervision of Certified Home Health

Aides is within the role Registered Nurse. "Therapy only" cases are routinely managed by a Registered Therapist, but first assessed by a registered nurse. The agency utilizes a case management system to provide comprehensive, coordinated care and services to patients served. Home Care Connect's goal has always been to treat every individual patient with compassion, respect and dignity. The agency has created an environment of teamwork and participation, with a focus on continual performance improvement. our highly skilled professionals provide home health care services, under a physician's supervision. Homecare services we provide include:
*Intermittent Care Services
*Skilled Nursing-Registered Nurses/Licensed Practical Nurses
*Attendant Care-Certified Home Health Aides
*Physical Therapy
*Occupational Therapy
*Counseling-Medical Social Worker
