Call Us 📞 (562) 438-5336 Eight years ago in a skilled nursing facility not so far away, our current team of Geriatric Care Managers (GCMs) began working together to serve the seniors in our community. Sharing a similar passion for enhancing the quality of life for aging adults and restoring balance and peace of mind to their families, we started Keen Home Care in 2005 wi

th the mission of helping even more families in ever more profound ways. It was an ambitious goal, but thanks to our more than 75 years of combined experience in assisting seniors and their families, and our comprehensive clinical backgrounds in nursing, therapy and social work, we feel we have made a positive, lasting difference in hundreds of people’s lives. We also believe that with every new success story, we gain new insights into the challenges aging presents to not only the senior, but their entire family. Being seen as “one of the family” to so many of our clients, we empathize with them and understand the incredible stress imposed by the tough and new choices they have to make, and that’s exactly why we founded Keen Home Care in the first place – we love to help. Now, over eight years later, as a seasoned team we are superbly equipped to deal with any situation, whether it’s a chronic condition, assisting with physical therapy or problematic behaviors related to Alzheimer’s or other dementias. Our diversity and experience also allows us to tailor our care to each client’s needs and budgets, which was our goal from the start. Keen Home Care has built the model of senior care we envisioned those many years ago, one built on helping families with compassion, integrity and accountability. We’re proud of what we do and you will be too.

Close to nine out of ten of the people in our care are living with some form of memory loss. This is not an abstract cause for us. This is the work we do every day.

This month, we at Keen Home Care are focused on sharing this message: A diagnosis is not a reason to pull someone out of their own life. Keeping people connected and included is the heart of good dementia care – something families have the most power to influence.

The goal of good transitional care is not to take over. It is to hold the hard part steady so your loved one can heal at home, where most people recover best.

Sometimes a few weeks of transitional care is all it takes to get someone on their feet again. Or maybe those weeks reveal that a little ongoing help would make everyone’s life easier.

Either way, you learn what your loved one actually needs by watching how the days go with a little professional support, so you don’t have to guess.

Sometimes families just need a little extra help in those few weeks post-hospital stay to ensure a smooth recovery, avoid setbacks, and assist with new needs.

Based on new needs, a care manager can ensure your loved one's home is safe to return to, as well as provide or source answers to questions you might not have even thought about.

And who do we call when something feels off at night, before it becomes a reason to go back to the hospital?

If someone you love is heading home soon, these are a few questions to consider ahead of time that can save you a great deal of stress.

If you are looking at a discharge date and feeling unsure about what comes next, that is exactly the moment worth a conversation. At Keen Home Care & Homes, we are always glad to help a family think it through!

A dementia diagnosis usually brings up the same fear…
The picture of a locked unit. A long hallway. A parent behind a door they cannot open themselves.

That fear is understandable. We have sat with hundreds of families who felt it the moment those words were said. But here is what years of doing this work has taught us:

A dementia diagnosis does not automatically mean a parent needs a locked environment. It means they need the right environment. Those are not the same thing.

Most people living with dementia do best with structure. A predictable routine they can trust. Faces they recognize every day.

The day a parent comes home from the hospital can feel like the finish line. After the tests, the waiting, and the worry, everyone is ready to exhale.

But the truth we have learned over many years is that home is often where the real work of recovery begins. The first two weeks after a discharge are one of the most fragile stretches in a person’s care.

If you are looking at a discharge date and feeling unsure about what comes next, that is exactly the moment worth a conversation.

At Keen Home Care & Homes, we are always glad to help a family think it all through.
