Independent pharmacy closures surged in 2025, heavily impacted by low reimbursements and steering practices by pharmacy benefit managers (PBMs). 1) PBM's are middlemen controlling drug pricing and plan reimbursements they frequently pay independents less than the drugs cost to acquire. 2) Preferred Network Exclusions; and 3) Patient Steering; PBMs frequently encouraged or mandated patients to use mail-order pharmacies or PBM-owned pharmacies, depriving local pharmacies of necessary prescription volume.
