To book your flu shot appointment head to our website - www.lopezislandpharmacy.com and select Vaccine Appointments, select your desired vaccine type and select a day and time. Then after you book your appointment fill out the vaccination consent form, there is a direct link to the form in the pop up you’ll see after booking as well as in your email confirmation.
