Pick up a packet at tomorrow’s back to school event.

Our Free Sport Physical day will be July 31st and August 1st at St. James Parish Hospital Rehab and Therapy Center. Physical packets can be picked up from LHS front office. Attached is a checklist for what students will need to complete. Also attached is just the physical form.

NOTE: You need to complete the whole packet to tryout and participate in sports at LHS (pick up from the front office).

From the first difficult steps after surgery to walking out of therapy with confidence, this patient showed what determination and hard work can accomplish.
