Vascular occlusion โ filler obstructing a vessel โ is the most serious filler risk. Vision loss has been documented in the literature. The estimated occlusion rate is roughly 1 in 5,000 syringes; vision loss is rarer still and tied to nose, glabella, and forehead.
Those high-risk zones contain ophthalmic artery branches. Filler entering intravascularly under pressure can travel retrograde to the central retinal artery. Lip filler specifically has essentially no published vision-loss cases โ lip vasculature feeds the facial artery, not ophthalmic branches.
