Vascular occlusion — filler obstructing a vessel — is the most serious filler risk. Vision loss has been documented in the literature. The estimated occlusion rate is roughly 1 in 5,000 syringes; vision loss is rarer still and tied to nose, glabella, and forehead.

Those high-risk zones contain ophthalmic artery branches. Filler entering intravascularly under pressure can travel retrograde to the central retinal artery. Lip filler specifically has essentially no published vision-loss cases — lip vasculature feeds the facial artery, not ophthalmic branches.

The tear trough is one of the most rewarding zones to treat correctly and most unforgiving treated poorly. The Tyndall effect occurs when hyaluronic acid sits too superficially in lower-eyelid skin under 0.5 mm — light refracts through the gel, creating a bluish bruise-like discoloration.

Lumps form when filler enters as concentrated boluses or when viscosity is too high for delicate tissue. Not every filler suits this region. Low G-prime, smooth hyaluronic acid for thin-skinned areas is the only correct choice — wrong product creates the problems patients fear.

Natural lip filler starts with respecting existing anatomy. Vermillion border, Cupid's bow morphology, upper-to-lower lip ratio, and lip projection relative to chin and nose determine what enhancement should look like for your specific face. There is no template that fits everyone.

The most common cause of unnatural results is over-volumizing the upper lip. The aesthetic ideal upper-to-lower ratio is approximately 1:1.6 — inverting it creates the duck-lip signature. Injecting too superficially at the wet-dry border produces a visible shelf or sausage roll.

The post-Botox alcohol restriction is grounded in real pharmacology, not liability. Ethanol is a vasodilator and a platelet aggregation inhibitor — it widens blood vessels and reduces clotting capacity. The consequence is bruising, the number-one cosmetic complaint after injection.

Botox uses 30 or 32 gauge needles puncturing skin, subcutaneous tissue, and muscle. Each needle track creates tiny vascular injuries that normally seal within minutes. With alcohol active, those micro-injuries clot slowly and blood spreads as a visible bruise.

Platysmal bands are vertical cords that tighten when you strain or talk, caused by hyperactivity of the platysma muscle — a thin sheet running from chest across the jawline. Botulinum toxin treats them effectively, but dysphagia concerns are anatomically informed and worth respecting.

The critical structures to avoid are the deeper cervical swallowing muscles: sternohyoid, thyrohyoid, and pharyngeal constrictors. These sit deep to the platysma, separated by fascia. Precise technique pinches the band, isolates it from deep structures, and injects superficially with 2–4 units per point.
