Diabetes care should not depend on income, insurance, language, or zip code.

The Diabetes Foundation is helping close the gap in care through FREE community-focused diabetes support that is not insurance based.

We continue providing: ✔ A1C screenings for early detection ✔ Diabetes education
✔ Community support groups ✔ Access to local diabetes resources

Because healthier futures begin with awareness, education, and access.

We’re excited to launch our new health literacy campaign—designed to empower children, adults, and families at risk or living with diabetes across New Jersey. Our easy-to-understand, image-focused posters and Diabetes TV are available in English, Spanish, and Haitian Creole, making vital information accessible to everyone. By embedding these resources in clinics and community centers, we’re already reaching over 200,000 people through their trusted providers. Together, we’re advancing health equity and helping priority populations take charge of their health. Have you seen our materials in your community? Let us know how they’re making a difference!

If you're age 35 or older, it's recommended to get your A1C checked—a simple test that measures your average blood sugar levels over the past three months and helps you stay on top of your health.

The Diabetes Foundation and North Hudson Community Action Corporation are partnering to offer confidential and convenient diabetes screenings, education, and free health resources. Eligible attendees may also receive diabetes supply boxes while supplies last.
