A patient recently came in with a concern that many people recognize right away:
💬“I’ve been having this pain that starts in my lower back and goes down my leg. Some days it’s manageable, other days it really interferes with my routine.”
They had already tried rest, stretching, and over-the-counter medication. It helped temporarily, but the pain kept returning.

This type of pattern is often associated with irritation of the sciatic nerve.
It doesn’t always present as severe pain.
Sometimes it’s discomfort, tightness, or a pulling sensation that comes and goes - but never fully resolves.

In these cases, the goal isn’t just short-term relief.
It’s understanding what’s contributing to the irritation and how to manage it more effectively over time.

Not every skin growth looks alarming at first.

This patient scheduled a visit after noticing a lesion that continued to grow, became irritated, and wasn’t healing normally. Skin lesions that crust, bleed repeatedly, become raised, or persist over time are common reasons people decide to get checked.

If you’ve ever tried an antidepressant and thought “this doesn’t feel right” - you’re not alone.
Some people don’t notice much improvement.
Others feel side effects almost immediately.
And sometimes it takes multiple attempts to find something that actually works.

One of the most common questions patients ask is:
“Why does the same medication work for someone else, but not for me?”
Part of the answer can be how your body processes medication.
This is where pharmacogenetic testing comes in.
It doesn’t “choose the right medication” or guarantee a result.
But it can provide insight into how your body may metabolize certain antidepressants - whether you process them too quickly, too slowly, or somewhere in between.

Sometimes skin changes happen gradually enough that people stop noticing them.

This patient came in after noticing that a spot had become darker, thicker, and different from surrounding moles over time. Not every skin change turns out to be serious, but changes in color, texture, shape, or size are usually worth evaluating.

One of the most important parts of dermatology is comparison — understanding what has stayed stable and what has changed.

In many cases, patients delay getting checked because the spot isn’t painful or doesn’t seem urgent. But skin concerns are often easier to evaluate and manage when addressed earlier rather than later.

After examination, a biopsy was performed and the tissue sample was sent for pathology evaluation. Early assessment allowed the issue to be addressed at an earlier stage, and the patient is doing well.

A skin check doesn’t automatically mean something serious is wrong. Most visits are simply about getting clarity and deciding whether monitoring, testing, or treatment is appropriate.

I-693 Medical Exam: what to bring and how to prepare
Preparing for an I-693 medical exam often feels more complicated than it actually is.
In most cases, it’s not the exam itself that causes stress - it’s not knowing what to bring or how to prepare.

The exam is required for many applicants applying for a green card from within the U.S. and must be completed by a USCIS-designated civil surgeon.
The process is structured, but preparation makes a big difference.

Most patients are asked to bring:
✔️a valid photo ID (passport, driver’s license, or state ID)
✔️vaccination records, if available
✔️any relevant medical history
✔️a list of current medications

If you’ve already started your paperwork, you can complete your personal information in Form I-693 before the visit - but it should only be signed in the presence of the doctor.

During the appointment, we review your medical history, perform a physical exam, and check vaccination status. Depending on current guidelines, certain lab tests may also be required.

If records are missing, it usually doesn’t stop the process - vaccines or additional testing can be done if needed.

One detail many people don’t expect:
your completed I-693 form is placed in a sealed envelope after the exam, and it must remain sealed when submitted to USCIS.
For most patients, the process is straightforward once everything is prepared correctly.

Most people do avoid seeing a dermatologist - not intentionally, but because other things feel more urgent.
Work, family, everyday responsibilities.
Or a thought like:

And over time, it just doesn’t happen.

When patients do come in, it’s often not just one concern.
It can be something that hasn’t gone away - acne that keeps coming back, redness that flares up, skin that feels irritated or sensitive for no clear reason.

Sometimes it’s a spot or growth - a mole, a small bump, a wart, or something new that doesn’t quite look like the rest of the skin.

Other times, it’s changes in pigmentation, hair thinning, or a condition like eczema or psoriasis that’s been managed on and off but never really evaluated.

Most moles and skin spots are harmless.
But some changes are worth paying attention to.

One of the simplest ways to monitor moles at home is the ABCDE rule - a guideline commonly used to recognize warning signs of melanoma.

Melanoma is one of the more serious forms of skin cancer, but when detected early, treatment outcomes are often very good.

Here’s what to look for:

🔸 A - Asymmetry
One half doesn’t match the other.

🔸 B - Border
Edges become uneven, blurred, or irregular.

🔸 C - Color
Different shades within the same spot, especially black, red, white, or blue areas.

🔸 D - Diameter
Larger than 6 mm can be worth monitoring, although melanomas can also be smaller.

🔸 E - Evolution
Any noticeable change in size, shape, color, or texture over time.

Other signs that shouldn’t be ignored include itching, bleeding, crusting, tenderness, or a spot that simply looks different from the others.

Many skin changes turn out to be benign.
The goal of a skin check is not to assume the worst - it’s to understand what you’re seeing and decide whether anything needs attention.
