GLP-1 medications can be a helpful tool for the right patient, but they are not magic and they are not the whole plan.

Real obesity care should also include looking at hunger and cravings, addressing insulin resistance, helping patients manage side effects, protecting muscle mass during weight loss, and making a plan for long-term maintenance.

Obesity is a chronic disease, and it deserves the same thoughtful medical approach as any other long-term health condition.

One of the reasons I created Elless is because this has been personal for me too.

I know what it feels like to struggle with weight, PCOS symptoms, hormone changes, and fatigue and feel like you are not getting real answers. I also know what it feels like to finally ask for help and be told to “just lose weight.”

That can feel embarrassing. It can feel defeating. And for a lot of women, it can make you not want to bring it up again.

Weight is not always just about willpower. For many people, there is a bigger metabolic and hormonal picture that deserves real attention.

That is why I believe in care that looks at the whole story, not just the number on the scale.

Metabolic dysregulation in obesity is far more than excess weight.

It involves disrupted signaling across adipose tissue, the gut, pancreas, liver, muscle, and brain.

Dysfunctional adipose tissue can drive:
chronic low-grade inflammation
insulin resistance
hyperinsulinemia
leptin resistance (when the brain stops responding normally to the hormone that signals fullness and stored energy)
impaired satiety signaling
and worsening metabolic disease over time

As adipocytes (fat cells that store fat and send metabolic signals) enlarge, they can become dysfunctional and further worsen these pathways.

That is why patients may experience persistent hunger, increased visceral fat, abnormal glucose regulation, fatty liver disease, PCOS, and difficulty maintaining weight loss.

Obesity is not just about excess fat mass.
It is also about abnormal fat function.
