Gastrointestinal specialists (GIs) have not kept up with the times; they have ignored the literature on Respiratory Reflux (aka LPR) … and have no tests or treatments for it.

GIs have spent the last 50 years defending their turf and protecting their incomes. GIs choose to know nothing about respiratory reflux, which happens to be ten times more common than esophageal reflux, which they call heartburn and GERD.

Exercise falls into four categories—aerobic, strength, balance, and flexibility—and any of them can trigger exercise-induced reflux in some cases. This typically happens for two main reasons: movements that compress the stomach (like sit-ups or certain yoga poses) and high-impact, bouncing activities (like running or jumping).

The good news is that reflux can often be managed with simple adjustments. Using lighter weights, avoiding breath-holding (which increases abdominal pressure), and skipping exercises done lying flat can help reduce symptoms. Preventative steps like acid-reducing medications, alginates, or even chewing gum may also provide relief.

Lastly, Respiratory Reflux (RR) and Laryngopharyngeal Reflux (LPR) refer to the same condition, though “RR” is preferred for being simpler and more inclusive of how reflux can affect the entire respiratory system.

Emerging insights point to silent respiratory reflux as a major, often overlooked factor behind a range of health issues. It may play a role in pneumonia—one of the leading causes of death among elderly nursing home residents—especially in cases where no clear infection is found. It can also contribute to or accelerate COPD, even in those who have never smoked.

Over time, this “silent” reflux may be linked to sleep apnea, often developing gradually over decades and, in some cases, being reversible with proper diagnosis. Even more concerning, it has been observed in patients with laryngeal and lung cancers—including lifelong nonsmokers—suggesting a deeper connection worth understanding.

The takeaway? Symptoms aren’t always obvious, but the impact can be significant. Learning to recognize and address respiratory reflux could be a key step in protecting long-term health.
