Most of us treat the annual physical as something that happens to us. We show up, get poked and prodded, nod along and leave. But the most valuable part of that visit is not the exam, it is the conversation, and you have more say in it than you think. A few good questions turn a routine checkup into a real plan for your year. Walk in with them written down, and make the visit work for you.

Nobody brings up constipation at dinner, which is part of why so many people quietly manage it for years with more prunes, more water and another box from the pharmacy aisle. Roughly one in six adults deals with it, and for many the root cause is a muscle coordination problem no laxative was ever going to fix. If your routine has stopped working, that is useful information, not a failure. Bring it to a primary care doctor or gastroenterologist at Cedars-Sinai Marina Hospital and let someone actually look for the cause.

Ulcerative colitis and Crohn's disease get grouped together so often that people assume they are basically the same thing. They are more like cousins than twins. Both are inflammatory bowel disease, both bring flares of pain, diarrhea and fatigue, but where and how deeply they inflame the gut is genuinely different, and that shapes everything from complications to treatment to surgery.

This is not a distinction to sort out on your own, since the two can look nearly identical from the outside and take a colonoscopy and imaging to tell apart. The encouraging part is that both, once properly diagnosed, are far more manageable today than they were even a decade ago. If your gut has been giving you trouble, the answer starts with the right diagnosis.

For years, carpal tunnel has been pinned on keyboards and mouse use, and it turns out the evidence for that is surprisingly thin. The bigger risk factors are things you would not think to blame: being a woman, since the tunnel in the wrist is naturally narrower, along with diabetes, thyroid conditions and even pregnancy. In other words, it often has more to do with your biology than your job. That matters because knowing your real risk means you can catch the early signs, the nighttime tingling, the hand you shake out to wake it up, before the numbness settles in for good. If that sounds familiar, it is worth mentioning to your doctor while it is still easy to treat.

Runners are trained to push through discomfort, which is usually a strength and, with a stress fracture, exactly the trap. Unlike a rolled ankle, a stress fracture does not announce itself. It starts as a small, specific ache on the bone that shows up during your run and fades with rest, so it is easy to write off and keep going. But that early window is precious, since backing off then lets the bone heal in weeks, while running through it can turn a hairline crack into a full break and cost you months. The rule of thumb is simple: a sharp, pinpoint pain that keeps getting worse is not something to train through. It is something to get looked at.

This is one of the few times we will say do not try to figure it out yourself. Chest pain from a panic attack and chest pain from the heart can feel almost identical, right down to the tightness, the racing pulse and the fear, and even trained doctors often need an ECG and bloodwork to be sure. That overlap is exactly why the safe move is always the same: if chest pain is new, severe or you are not sure, treat it as your heart and get emergency help. None of this dismisses anxiety, which is real and worth caring for. It just means the dangerous possibility has to be ruled out first. Being checked and told your heart is fine is the best possible outcome, and it is never a waste of anyone's time.

Here is a condition that a quarter of adults may have right now without feeling a thing. Fatty liver disease has quietly become the most common liver problem there is, rising in step with weight, type 2 diabetes and cholesterol, and it rarely announces itself. For many people it stays mild, but in some it slowly leads to real liver damage, which is exactly why silent does not mean safe. The encouraging part is that the liver is remarkably good at healing. Caught early, this is one of the most reversible conditions out there, and modest changes to weight, movement and diet can genuinely turn it around. If you have not thought about your liver in a while, it is a good thing to ask about at your next visit.

Hip replacement recovery has a way of surprising people from both sides. Many are shocked at how quickly they are up and walking, sometimes the very same day, and how much less it hurts than they feared. Then they are surprised again a few weeks later, when they expected to feel completely normal and instead hit the long, less dramatic middle of recovery. Both are the real story. The early weeks move fast, and the last stretch, getting back full strength and that natural, forgettable feeling in the joint, takes patient months, not weeks. The people who do best keep up the physical therapy and give themselves permission to heal on the body's timeline, not the calendar's.

Going gluten-free has become so common that it is easy to forget there is a serious medical condition underneath it all. Celiac disease is not a preference or a sensitivity, it is an autoimmune disease that damages the small intestine, and it needs a proper diagnosis and strict, lifelong care. Gluten sensitivity is real too, but it does not carry that same internal damage. The single most useful thing to know is this: if gluten seems to bother you, get tested for celiac before you cut it out, because going gluten-free first can hide the very thing the test is looking for. Guessing is the one approach that can leave a real problem undiagnosed.

If you or someone you love is going through cancer treatment, you have probably heard a hundred rules about what to eat and what to avoid. A lot of them are not just wrong, they are backwards. The fear that sugar feeds cancer, or that a strict diet can starve it, leads many people to eat less at exactly the time their body needs more. During treatment, keeping your strength up with enough food and protein is one of the most helpful things you can do, and it matters more than any superfood or detox. If food feels complicated right now, that is what an oncology dietitian is for. The goal is not a perfect diet. It is nourishing you through this.
