So many women spend years feeling off, exhausted no matter how much they sleep, gaining weight without changing anything, feeling anxious or foggy or just not like themselves. They get told their labs are normal and leave the doctor's office with no real answers. And so often the missing piece comes down to one small gland in your neck.

Your thyroid controls your metabolism, your energy, your mood and even your hair growth. When it's underactive, a condition called hypothyroidism, everything in your body slows down. When it's overactive, called hyperthyroidism, everything speeds up in ways that feel just as overwhelming.

Ask any orthopedic specialist what their busiest season is and the answer is almost always summer. Not because people are doing anything wrong. Because they are finally doing everything they love again.

The transition from a slower few months to full summer activity is where knees tend to struggle most. Cartilage and tendons need time to adapt to new demands. When that adjustment period gets skipped, the joint speaks up.

Two of the most common conditions we see this time of year are patellofemoral pain syndrome and patellar tendinitis. Patellofemoral pain, often called runner's knee, causes aching around or behind the kneecap and tends to flare up with stairs, squatting or sitting for long periods. Patellar tendinitis is inflammation of the tendon connecting the kneecap to the shinbone and usually shows up as pain just below the kneecap during or after activity. Both are overuse injuries. Both are preventable.

IT band syndrome is another one worth knowing. The iliotibial band runs along the outside of the thigh from the hip to the knee and when it gets tight from repetitive movement it causes sharp pain on the outer side of the knee.

Cyclists and runners are particularly susceptible at the start of a new season.
Worth paying attention to regardless of activity: swelling that does not go down after rest, pain that gets worse on stairs or inclines, a clicking or locking sensation that feels new and any instability that makes you feel like your knee might give out. These are not things to push through.

The best defense is a strong foundation. Your quads, hamstrings and glutes absorb a significant amount of the load your knee joint would otherwise take on its own. Targeted strengthening before you ramp up activity makes a measurable difference. Straight leg raises, wall sits, clamshells and single leg squats are all effective and require no equipment. Swimming and cycling are excellent complements because they build the surrounding muscles without adding compressive stress to the joint.

Proper footwear matters more than most people realize. Worn out soles change the way force travels through your foot and up into your knee. If your shoes have seen better days, the start of summer is a good time to replace them.

Hydration plays a role too. Synovial fluid, which lubricates your joints, depends on adequate hydration to do its job. Especially in the Southern California heat, drinking enough water is not just good for your performance. It is good for your joints.

A little preparation now means more beach volleyball, more hiking and more of whatever makes a Southern California summer worth having.

Los Angeles does not ease you into summer. One week it is overcast and mild and the next the temperature climbs and stays there. For anyone who stays active outdoors that shift requires a real adjustment.

Heat exhaustion is more common than most people realize. It does not only happen to athletes or people working in direct sun for hours. It can happen on a morning run, a long bike ride or even a walk along the beach if conditions are right and you are not paying attention.

Understanding the difference between heat exhaustion and heat stroke is important. Heat exhaustion is the warning. Symptoms include heavy sweating, cold and clammy skin, a fast and weak pulse, nausea, muscle cramps, dizziness and fatigue. The response: move to a cool place immediately, loosen clothing, apply cool wet cloths to the skin and sip water. If symptoms do not improve within 15 minutes or the person vomits, seek emergency care.

Heat stroke is the emergency. It occurs when the body's core temperature rises above 104 degrees Fahrenheit and the cooling system fails. Symptoms include confusion, slurred speech, a rapid and strong pulse and skin that may be hot and dry in classic heat stroke or still moist in exertional cases. Heat stroke requires a call to 911 immediately. While waiting for help, move the person to a cool environment and apply ice packs to the neck, armpits and groin to lower core temperature as quickly as possible.

Humidity is a factor most people overlook. High humidity prevents sweat from evaporating efficiently, which is the body's primary cooling mechanism. A day that feels manageable by temperature alone can still carry significant heat illness risk when humidity is elevated.

A few habits that make a meaningful difference: drink water consistently throughout the day rather than waiting until you are thirsty, because thirst is already a sign of mild dehydration. Avoid peak sun hours between 10am and 2pm when heat and UV exposure are at their highest. Wear lightweight breathable clothing and take rest days seriously.

Electrolytes matter too. Sweating depletes sodium, potassium and magnesium. Plain water alone does not fully replace what you lose during intense activity in the heat. A sports drink or electrolyte supplement after a long session helps your body recover more effectively.

Marina del Rey is one of the best places in Southern California to get out on the water. On any given summer morning, the harbor reflects that. Stand up paddleboarding has become one of the most popular ways to stay active along the coast and it is easy to see why. It is fun, it is scenic and it works your entire body.

What most people do not realize is how much it works your body until the morning after.

The shoulder takes the most punishment. Repetitive paddling puts significant strain on the rotator cuff, especially when form breaks down toward the end of a long session. Lower back pain is almost as common, particularly in people who rely on their spine to stay balanced rather than their core. Both tend to sneak up on you.

Before you get back on the board this season, build some core strength on land first. Planks, dead bugs and single leg exercises will make you a more stable paddler and take pressure off your lower back. On the water, focus on driving each stroke with your whole body rather than just your arms.

Give yourself rest days too. Tendons recover slower than muscles and the start of the season is when injuries happen most.

There is a reason cyclists from all over Los Angeles make the trip to Marina del Rey. The stretch of coastline from the marina down to Santa Monica is one of the most beautiful rides in the city and come June it is packed.

If you are getting back on the bike after a slower few months, your body is going to feel it. That is completely normal. What is not normal is pushing through pain because you assume it is just rust.

The two most common mistakes returning cyclists make are skipping the warm up and ramping up too fast. Your hip flexors, knees and lower back need time to remember what they are doing. Start with shorter rides and build from there. Dynamic movement before you clip in makes a real difference.

During your ride, pay attention to how your knees track. Pain on the outside of the knee is often a sign your seat height is off. Pain below the kneecap usually points to overuse. Both are worth addressing before they sideline you for the summer.

Cycling is one of the best things you can do for your joints. Do it right and your body will thank you all season long.

The goal is not 100. It is being strong at 80.

There is a quiet shift happening in how the most health-conscious people think about aging and if you live in Marina del Rey chances are you are already living it without realizing it.

For decades the conversation around longevity focused on lifespan: how many years you have. But the conversation that matters more is healthspan: how many of those years you spend feeling genuinely well. Strong, clear-headed, active and present.
