The process to become a patient is as follows:

To become a new patient, we require a New Patient Application to be completed and returned before scheduling.
Please allow 72 hours for review and for your information to be entered into our EMR system.

You can access the application:

• On our website → Patient Resources > Become a Patient > New Patient Application
