Meet Anna Zarlingo, FNP — she leads our Spravato and ketamine clinic.

Anna came to advanced psychiatric care the long way around. A computer science degree first, then twelve years as an emergency department nurse, then family nurse practitioner.

That ER background matters more than it might sound. Spravato and ketamine treatment are given under observation, and they ask for a clinician who monitors closely, reads a room quickly, and stays steady when something needs attention. Anna spent over a decade doing exactly that.

She now runs the day-to-day operations of both programs alongside Dr. Nigam, working with people whose depression has not responded to standard antidepressants. She is board certified through both the ANCC and the AANP.

Patients taking semaglutide are already asking at medication management visits whether the mood changes they notice are real. The short answer: for semaglutide specifically, the evidence is genuinely interesting, though not every GLP-1 drug showed the same effect.

A 13-year Swedish cohort study found semaglutide linked to fewer psychiatric hospitalizations and less mental health-related sick leave; liraglutide showed a smaller benefit, and two older GLP-1 drugs showed none. These drugs are not FDA-approved for depression or anxiety, the study was observational, and it is not a substitute for psychiatric care.

A new kind of ADHD medication called centanafadine is approaching an FDA decision, and the mechanism behind it is genuinely different from most current options.

Most ADHD medications target dopamine and norepinephrine. Centanafadine targets all three: dopamine, norepinephrine, and serotonin. That triple-reuptake profile may address emotional reactivity alongside attention and impulse control.

It is also a non-stimulant, meaning it would not carry Schedule II controlled-substance status. For patients who have not done well on stimulants, or for whom a non-controlled option matters, that distinction is significant.
