🩺 What You’ll Do
As a Hospice RN, you will play a vital role in delivering compassionate, patient-centered care:
Manage a dedicated patient and family caseload, including:
Assessments
Eligibility determination
Care planning, implementation, and evaluation
Conduct holistic assessments (physical, psychological, social, spiritual) and coordinate appropriate interventions
Collaborate with physicians, interdisciplinary teams, and external partners to ensure optimal care
Perform regular patient visits to reassess needs and update care plans
Provide compassionate end-of-life care, including clinical pronouncement and family support
Maintain accurate and timely documentation (charts, notes, care plans)
Supervise and document Home Health Aide services in compliance with regulations
✅ Qualifications
Graduate of an accredited RN program
Active Registered Nurse license in the state of practice
Current BLS and First Aid certification
Knowledge of quality assessment and assurance practices
Valid driver’s license and proof of insurance
