Morning back stiffness isn't one thing. The pattern of when it improves — or doesn't — tells us a lot about what's actually going on.

PATTERN ONE: stiff when you wake up, noticeably better within 30 minutes of moving around.

This usually points to a mechanical issue. Discs, joints, and muscles lose hydration overnight and rehydrate as you start moving. The stiffness is your body's recovery system catching up. This pattern tends to respond well to chiropractic care, spinal decompression, and targeted movement.

PATTERN TWO: stiff for an hour or more, and movement doesn't help. Sometimes it wakes you up in the second half of the night.

This can point to inflammatory conditions — things like ankylosing spondylitis or autoimmune arthritis. These require different testing and a different treatment approach entirely. They're commonly missed for years because patients (and clinicians) treat them like ordinary back pain.

Most people have been told their stiffness is "just back pain." It isn't. The pattern matters. The treatment depends on it.

"Strong abs" and "strong core" aren't the same thing. And the difference matters more than you'd think.

Most people training their core are training the wrong muscles. Crunches, sit-ups, Russian twists — these work the surface-level abs you can see in the mirror. Those aren't the muscles that protect your spine.

Your real core sits underneath:

✓ Transverse abdominis — your body's natural weight belt that wraps around your midsection
✓ Multifidus — tiny muscles that lock each vertebra in place
✓ Pelvic floor — the foundation that ties it all together
✓ Diaphragm — yes, your breathing muscle is part of your core

These four work as a team to stabilize your spine every time you bend, twist, lift, or even sneeze. None of them get trained by sit-ups.

This is why so many people "have a strong core" and still throw their back out tying their shoes. They've been training the wrong layer — and in some cases, crunches and sit-ups can actually make disc issues worse.

A real core program looks different. It's slower, more controlled, and targets the deep stabilizers your spine actually relies on.

Your back hurts more during your worst weeks at work. That isn't in your head. It's in your nervous system.

When you're stressed, your body shifts into "fight or flight." This is useful in short bursts and harmful in long ones. The muscles around your spine — particularly the upper traps, the neck, and the lower back — contract and stay contracted. Hours of this. Days of this. Years of this.

That alone causes pain.

But there's a second mechanism most people don't know about. Chronic stress raises levels of cortisol and other stress hormones, which increase inflammation throughout your entire body. If you already have a disc issue, joint dysfunction, or nerve irritation — that inflammation makes the existing problem hurt more, heal slower, and flare up more often.

So when stress goes up, your back pain goes up. Even if nothing physically changed. Your body's pain volume just got turned up.

This is why treating the back without addressing the system around it tends to bring short-term relief and long-term frustration.

A real treatment plan addresses both sides. Adjustments and decompression release the physical tension. Recovery habits — sleep, breathing, mobility, movement — calm the nervous system itself. Together, they break the cycle.

What happens after college sports end?
For a lot of athletes, the hardest part is figuring out what’s next.

For Dr. Sam Marri, that answer became HYROX, half marathons, and continuing to challenge herself in new ways. But one of the biggest lessons she learned along the way was realizing she didn’t have to know everything on her own.
