Getting older doesn't mean getting weaker. In fact, strength training becomes even more important after age 40 to help maintain muscle mass, support joint health, improve balance, and preserve independence.

Here are 3 foundational exercises that deserve a place in your routine:

1๏ธโƒฃ Sit-to-Stand

This exercise mimics one of the most common daily activitiesโ€”getting up from a chair.

โœ… Builds leg and glute strength
โœ… Improves balance and mobility
โœ… Enhances independence with daily activities

Challenge: Perform 10 repetitions without using your hands.

2๏ธโƒฃ Carrying Exercises

Think farmer's carries, grocery bag carries, or suitcase carries.

โœ… Improves grip strength
โœ… Strengthens your core and postural muscles
โœ… Makes everyday tasks feel easier

Challenge: Walk 30-60 seconds while carrying a challenging weight with good posture.

3๏ธโƒฃ Step-Ups

A simple but effective way to build lower body strength and stability.

โœ… Strengthens the hips, glutes, and legs
โœ… Improves stair-climbing ability
โœ… Enhances balance and coordination

Challenge: Perform 2-3 sets of 8-12 repetitions per leg.

Did you know that pelvic floor dysfunction is more common in individuals with Ehlers-Danlos Syndrome (EDS)?

Because EDS affects collagen and connective tissue throughout the body, the structures that support the bladder, bowel, and pelvic organs can be impacted too.

Common pelvic floor concerns may include:
๐Ÿ”น Pelvic organ prolapse
๐Ÿ”น Urinary leakage with coughing, sneezing, or exercise
๐Ÿ”น Urinary urgency and frequency
๐Ÿ”น Constipation or difficulty with bowel movements
๐Ÿ”น Pelvic pain
๐Ÿ”น Pain with in*******se

One of the biggest misconceptions is that symptoms are always caused by a "weak" pelvic floor. In reality, many people with EDS develop increased muscle tension and guarding as their bodies attempt to create stability around hypermobile joints and connective tissues.

The pelvic floor is part of a larger system. Addressing breathing mechanics, pressure management, strength, coordination, and whole-body stability often leads to better outcomes than focusing on Kegels alone.

Recovering from an injury sucks! Things that were once easy become a struggle. Stiffness sets in and causes soreness. Activities have to be modified.

But, the process is necessary and beneficial. This is what happens when the PT needs PT, and I'm not always a good patient because I want to be better NOW!

๐Ÿ‹๏ธโ™€๏ธ Does lifting heavy weights cause pelvic organ prolapse?

Many women have been told to avoid heavy lifting to protect their pelvic floor. But the research tells a more complex story.

A cross-sectional survey by Forner et al. (2020) examined symptoms of pelvic organ prolapse in women who participated in heavy weightlifting. Interestingly, women who reported lifting less than 15 kg were more likely to report symptoms of pelvic organ prolapse than women lifting more than 50 kg.

What does this mean?

It doesn't mean heavy lifting prevents prolapse. It also doesn't mean heavy lifting causes prolapse.

It does suggest that the relationship between strength training and pelvic floor health is more nuanced than simply "heavy weights are bad for your pelvic floor."

As we age, maintaining muscle mass becomes increasingly important. Sarcopenia (age-related muscle loss) is associated with increased risk of falls, frailty, functional decline, and loss of independence. Avoiding resistance training out of fear may have unintended consequences for long-term health.

The goal isn't to avoid loading the bodyโ€”it's to build strength in a way that works for your body.

Pelvic floor symptoms during exercise are often an invitation to assess factors such as breathing mechanics, pressure management, exercise programming, recovery, overall strength, and individual risk factorsโ€”not necessarily a sign that exercise should stop.

Strong muscles support healthy aging.
Strong muscles help reduce frailty.
And a strong pelvic floor deserves the same evidence-based approach as the rest of the body.

As we age, we naturally lose muscle mass and strengthโ€”a condition known as sarcopenia. But this isn't just about feeling weaker. Sarcopenia can have a major impact on your overall health, independence, and quality of life.

โš ๏ธ Sarcopenia may increase your risk of:

๐Ÿ”น Pelvic Organ Prolapse
The pelvic floor is made of muscles and connective tissue. When muscle strength declines, pelvic support may be compromised, increasing the risk of pelvic floor dysfunction and prolapse.

๐Ÿ”น Falls & Injuries
Less muscle strength means decreased balance, slower reaction times, and reduced ability to catch yourself if you stumble.

Every decade after age 30, adults can lose 3โ€“8% of their muscle mass if proactive steps aren't taken. The earlier you invest in muscle health, the better your long-term outcomes.

When people think about uterine fibroids, they often think about heavy periods and menstrual cramps.

But fibroids can also contribute to musculoskeletal pain and pelvic floor dysfunction, which means symptoms may show up far beyond the uterus.

๐Ÿ”น Low Back Pain
Larger fibroids can create pressure within the pelvis and abdomen, contributing to low back pain. In some cases, fibroids may even irritate nearby nerves, leading to pain that radiates into the hips or legs.

๐Ÿ”น Pelvic Pressure and Pelvic Floor Symptoms
Fibroids can create a feeling of heaviness, pressure, or fullness in the pelvis. This increased pressure may contribute to urinary urgency, urinary frequency, constipation, difficulty emptying the bladder, and pelvic discomfort.

๐Ÿ”น Pain With In*******se
Depending on their size and location, fibroids may contribute to discomfort or pain during intimacy.

๐Ÿ”น Hip, Buttock, and Leg Symptoms
Some women report symptoms that mimic sciatica, including pain into the buttock or leg. While not the most common presentation, large fibroids can occasionally compress surrounding structures and nerves.

๐Ÿ”น Core Dysfunction
Persistent pelvic pain, abdominal pressure, and altered movement patterns can affect how the core and pelvic floor muscles function together, potentially contributing to muscle tension, weakness, or coordination issues.

As physical therapists, we often see women who have been treating "back pain" or "pelvic floor symptoms" without realizing that fibroids may be an underlying contributor.
