Can you actually fill in lost gum tissue with a syringe? 🦷💉

In this Q&A, Dr. Choi and Dr. Koo discuss “gum filler” for black triangles — the open spaces that can develop between teeth when the interdental papilla and underlying bone are lost.

While injectable fillers have been proposed as a way to temporarily plump up deficient papillae, the doctors explain why they generally wouldn’t recommend relying on filler as a predictable solution. Rebuilding lost papilla around natural teeth can be extremely challenging, and aggressive surgical attempts at papilla regeneration can sometimes make the situation worse.

They also discuss:

• How adult tooth movement can affect the surrounding bone and gums

For a month following her All-on-4 surgery, this patient stopped wearing her temporary upper denture. Without that denture in the mouth, her bite collapsed, her vertical dimension dropped, and her jaw muscles — masseter and temporalis — adaptively shortened around the new position. She arrived with bilateral myofascial pain and an opening so limited our oral surgeon Dr. Han (Dr. Sung Han DDS, MD ) couldn’t visualize the upper arch.

Under conscious sedation, Dr. Han relaxed the musculature, performed passive stretching, and seated the temporary denture to restore her vertical dimension and start the rehab process.

Watch him passively open the jaw and feel for the end point — a springy end feel means muscle, not a displaced disc. That’s the distinction that drives the whole treatment plan.
Restore the vertical dimension, and the muscles can re-adapt.

Why We Often Choose a Two-Stage Approach for Implants at Our Office!

There’s a growing trend in implant dentistry toward doing everything in a single appointment — pulling a failing tooth, placing the implant, and attaching a healing abutment all at once. When it works, it’s a great outcome: fewer visits, less chair time, and a faster path to your final crown.

But one-stage isn’t inherently worse — it’s a matter of managing risk and control. The moment an abutment is attached, the implant site is exposed to oral bacteria, and any pressure from chewing or food can disrupt an implant that hasn’t fully fused to the bone yet. Healing varies a lot from patient to patient, and some factors raise that risk — smoking and diabetes, for example, are both known to slow healing and increase the chance of complications.

There’s also the gum tissue itself. Thicker, keratinized gum tissue around an implant generally offers better long-term protection, and sometimes that’s easier to achieve once the site has fully healed rather than trying to manage it during the same appointment as placement.

That’s why, especially for patients we consider higher-risk — smokers, diabetics, and others with factors that can compromise healing — we often choose a two-stage approach: place and bury the implant first, let it heal and stabilize, then come back to address the gum tissue and expose the implant when everything is more predictable.

It’s not that one method is right and the other is wrong — it’s about matching the approach to the individual case, and for higher-risk patients, taking a bit more time gives us more control over the long-term outcome.

In this video, our star prosthodontist Dr. Shahrukh Syed walks through the exact steps he takes to design a patient’s first set of implant-supported temporary teeth — built to match their ideal smile from day one.
