Perimenopause is a deeply personal experience. Hormonal changes can affect sleep, mood, cognition, and social connection, knowledge and support can make all the difference.

Research continues to show that staying socially engaged, nurturing meaningful relationships, and maintaining a hopeful outlook can positively impact both physical and mental health during this transition.

You are not alone. With the right support, this chapter can be one of growth, resilience, and renewed well-being. 💛

Perimenopause doesn’t always come with a clear roadmap and neither do the questions that come with it.

That’s why is tackling some of the most common and nuanced questions providers ask about navigating hormonal changes during this transitional stage. From hormone testing and therapy options to symptom patterns and clinical considerations, these discussions help bring greater clarity to a complex topic.

Scroll and listen to the questions below:
1️⃣ Does the DUTCH Test only measure progesterone metabolites from an oral dose?
2️⃣ Is EstroGel a good option given that patches are currently in short supply?
3️⃣ Isn’t DIM not recommended in late perimenopause, even if estrogen is still erratic, because it lowers estradiol levels? Can a woman take DIM occasionally such as when she feels estrogen dominant with breast tenderness and then stop?
4️⃣ Do we know why some women cruise through perimenopause while others struggle with so many symptoms?
5️⃣ Are blood clots part of heavy me**es or does that typically indicate something else?
6️⃣ Is it best to cycle progesterone 14 days on and off? I’ve recently learned that taking it continuously each night can downregulate estrogen receptors.
7️⃣ What days of the cycle do you use a progesterone suppository?
8️⃣ Why not add pregnenolone daily in addition to pulsing progesterone in the second half of the cycle?

Stress is not simply an emotional experience, it is a physiologic process that influences metabolism, sleep, immune function, cognition, and hormone regulation.

When the stress response system is activated continuously, the body shifts from adaptation to dysregulation. Over time, this persistent demand can alter cortisol rhythms, disrupt recovery, and reduce the capacity to respond effectively to future challenges.

Resilience isn’t the absence of stress. It’s the ability to recover from it.

Perimenopause is one of the most significant hormonal transitions a woman will experience, yet too many women navigate it without the support, education, or clinical guidance they need.

Hormonal fluctuations during this phase can affect everything from metabolism and body composition to mood, energy, and overall well-being. Recognizing the signs of perimenopause is essential, but so is having knowledgeable healthcare providers who can help women understand what’s happening and identify the best path forward.

On The DUTCH Podcast, discussed the complexities of perimenopause, including the role of estrogen in insulin sensitivity, why symptoms are often overlooked, and the importance of better education for both women and clinicians. DUTCH testing can provide deeper insight into hormonal health and support more personalized care.

Listen as answers a question submitted during our recent webinar on navigating perimenopause:

Do you have recommendations for patients who suspect they are in perimenopause but have had a hysterectomy?

Hormonal shifts during this stage of life can be complex, nuanced, and highly individualized. Symptoms may evolve over time, hormone patterns can fluctuate significantly, and without menstrual cycles as a reference point, identifying where a patient is in the transition can become even more challenging.

That’s why conversations like these matter. Our goal is to help providers feel more confident navigating the uncertainty of perimenopause with practical clinical insights and evidence-informed guidance.

Personalized hormone therapy goes beyond prescribing the right dose. It also requires understanding how different testing methods reflect what’s happening clinically, and when certain results may be more misleading than meaningful.

As conversations around hormone therapy monitoring continue to evolve, it’s important to challenge common misconceptions surrounding creams, gels, and saliva testing while examining what the clinical data actually shows about dosing accuracy across different testing modalities.

Thoughtful use of serum, urine, saliva, and other testing approaches can help providers better match the right test to the right clinical question, ultimately helping reduce the risks of under or overdosing and optimize patient care.

Advancing best practices in integrative medicine means continually revisiting the evidence, refining our understanding, and remaining open to where the science leads.

“Hormones are truly the art of functional medicine…” 🎨

In this episode of The DUTCH Podcast, , CMO of , .jaclynsmeaton, and DUTCH founder and CEO, Mark Newman, discuss the importance of monitoring hormone therapy accurately by relying on clinical evidence over established practice.

Inspired by our recent peer-reviewed publication in Frontiers in Reproductive Health, the discussion explores the validation of serum testing over saliva for monitoring transdermal testosterone therapy, how DUTCH Testing fits into that equation, and what that means for patient care.

Their discussion also covers:
- Common misconceptions about how creams and gels show up in saliva testing and why the numbers can be misleading
- How DUTCH testing, when paired with serum testing, aligns with clinical outcomes
- Why advancing best practices in integrative medicine requires continually revisiting what the evidence supports
