Earlier this week, we had the chance to attend an Ultherapy event in Jupiter, and I loved getting to spend the day learning, connecting, and continuing to deepen our knowledge of a treatment we use so often in the practice.

If you’ve been looking in the mirror and thinking, “I want a little more shape, but I do not want surgery,” this may be worth exploring. This patient came to me with two main concerns: skin laxity and loss of volume.

We focused on improving both shape and skin quality. We stimulated collagen but using Radiesse filler which functions in the skin as a biostimulator.

Why don’t I typically inject filler directly into the nasolabial fold?

Because the fold itself isn’t always the real issue.

As we age, we lose volume and support in other areas of the face, especially through the cheeks and midface. That change can make the nasolabial folds look deeper or more noticeable over time.

Depending on the patient, I may recommend treatments like skin tightening with Ultherapy or RF microneedling, surgical options like a facelift, or using filler strategically in other areas of the face to restore lost volume and soften the appearance of the fold.

And it’s important to remember: some people naturally have more prominent nasolabial folds because of their anatomy, and there is absolutely nothing wrong with that. For others, they become more apparent with age.

The goal should never be to simply “fill the line.” It’s to understand why it’s there and choose the treatment that makes the most sense for your face.

Find an injector who treats the cause, not just the symptom.

I wanted to talk about what is likely happening here, because this is a really good example of why lower-face Botox requires such a strong understanding of facial anatomy.

From the way her smile is pulling, it looks like the Botox may have affected a neighboring muscle that wasn’t intended to be treated. Even a small amount of product placed in the wrong spot (or spreading into an adjacent muscle) can temporarily change the way someone smiles.

Of course, I can’t know exactly what was injected or where just from a video, but this is why injection placement, dosing, and understanding how each muscle contributes to facial movement matters so much.

Lower-face Botox can give beautiful, subtle results when it’s thoughtfully placed and tailored to the patient’s individual anatomy.
