I'm going to say something a lot of psychiatrists won't say publicly: I take medication—a small dose of Lexapro—and it's been one of the most meaningful decisions of my adult life. I'm not numbed out. I'm not a different person. I'm just... less at war with my own nervous system. The stigma around psychiatric medication is hurting people, not because medication is always the answer (it isn't), but because the fear of it is keeping people from a tool that could genuinely change their quality of life. I'm not trying to push medication; I'm trying to help you make a grounded, fear-free decision about whether it's right for you. 💬 Drop your questions about medication in the comments.

Too many people are treating exhaustion like a personality trait. or accepting a baseline of meh as "just adulthood." It doesn't have to be that way. Drop a 💙 below if this resonated.

After 10+ years of practice, here's what I know about ADHD: The people I treat with ADHD aren't distracted. They're differently regulated. They don't have an attention deficit—they have an attention inconsistency. And under the right conditions (urgency, novelty, passion), they focus like lasers. Studies show ADHD traits are 5–10× more common among CEOs, innovators, and entrepreneurs. Disruption is a feature, not a bug—when it's channeled. If you've spent your life being told you're too much, too scattered, too intense, I want you to know: that intensity is a superpower waiting for the right container. 📬 Subscribe to my Substack: https://doctoralexmd.substack.com

The most liberating thing I tell patients is this: you are not your reactions. Your reactions are the output of a system—imagination on one side, impulse control on the other—that's been shaped by biology, sleep, stress, and history. When the engine is bigger than the brake, things get loud. And the fix isn't just "try harder to control yourself." It's understanding the system—and then strengthening the brake. Save this for the next time you spiral and need a framework instead of self-blame. 💙

Feelings are a significant ADHD symptom, yet nobody screens for it. Up to 70% of adults with ADHD struggle to regulate their feelings, but it isn’t included in the diagnostic criteria. A hidden effect is broken promises. First to other people, then to yourself. Over time, this can drain your confidence and leave you feeling like an adult child, but there is help! This is the metaphor that changes how my patients think about why they're stuck. You've done the work. You've tried the mindset shifts, the habits, the therapy. And it worked—until it didn't. Until life got steep. That's not a failure of willpower or effort. That's what happens when biology is running the show and we're only addressing psychology. The answer isn't to abandon self-help. It's to add the missing layer: your biology.
