Where did all the good physical therapy clinics go?

We spend so much time talking about what NEW GRADS want in their first job…but maybe we need to start asking the therapists who have been out 5, 10, or 15 years what happened to them.

New grads enter this profession excited, optimistic, and ready to help people.

And unfortunately, that can make them the perfect target for high-volume clinics.

16–20 patients a day. Double and triple booking. Documentation through lunch. Notes after work. Productivity constantly hanging over your head.

At first, you might think: “This is just what being a PT is.”

But what happens after 5 years of it?

How many great therapists have already left outpatient PT—or the profession altogether—because the system burned them out?

We keep talking about a physical therapist shortage while continuing to feed new graduates into the same system that burned out the generation before them.

Maybe we don’t have a PT shortage.

Maybe we have a GOOD CLINIC shortage.

A big thank you to the coaches and staff UC Merced for putting on such a great soccer camp! ⚽💙

It’s been so much fun watching our kids grow in confidence, improve their skills, and most importantly, enjoy the game they love. The time, energy, and encouragement you pour into these young athletes makes a real difference.

We’re grateful for programs like this that help foster a love of sports, teamwork, and lifelong healthy habits. Looking forward to seeing everyone continue to grow both on and off the field!

Day 30 — and first, thank you for following along with this series. 🙏

If there’s one message I want to leave you with as a physical therapist, it’s this:

Your body is more adaptable than you think.

Pain, stiffness, and injury can feel limiting—but they don’t define your long-term potential.

Too often, people feel like they need perfect conditions before they can start moving again:

❌ No pain
❌ Perfect program
❌ Perfect timing

But progress usually doesn’t work that way.

Instead, it comes from:

✅ Consistent movement
✅ Gradual progression
✅ Building confidence over time
✅ Learning to work with your body—not against it

Recovery isn’t about doing everything perfectly.

It’s about doing the next right thing… again and again.

Small steps matter more than perfect ones.

And over time, those small steps add up.

If this series helped you, share it with someone who needs a reminder that their body is capable of more than they think.

What’s the best exercise? 👇

People ask this question all the time.

The answer?

The best exercise is the one you’ll actually do consistently.

Walking.
Strength training.
Cycling.
Swimming.
Yoga.

All of them can be great choices.

The problem isn’t usually finding the “perfect” exercise.

The problem is finding something you can stick with long-term.

Consistency is what drives results.

Not perfection.

Not the newest workout trend.

Not the most complicated routine.

The exercise program that works best is the one that fits your life and keeps you moving regularly.

Because health isn’t built in a day.

It’s built through habits repeated over time.

Find movement you enjoy.

Make it part of your routine.

And keep showing up.

You don’t have to be pain-free to start moving. 👇

One of the most common things people tell me is:

“I’ll get back to exercising when my pain is completely gone.”

While that sounds logical, it’s not always the best approach.

For many conditions, appropriate movement can actually be an important part of recovery.

Movement can help:

✅ Maintain strength
✅ Improve mobility
✅ Build confidence
✅ Support function
✅ Reduce fear of activity

That doesn’t mean ignoring pain.

And it doesn’t mean pushing through severe symptoms.

It means finding the right starting point and gradually progressing from there.

Waiting for everything to feel perfect can sometimes delay recovery longer than necessary.

Remember:

Recovery is often about doing what you can, not waiting until you can do everything.

Small steps still count.

Pain does not always equal damage. 👇

This is one of the most important concepts we teach in physical therapy.

Many people assume that if something hurts, they must be causing harm.

But pain is much more complex than that.

Pain can be influenced by:

✅ Tissue irritation
✅ Stress
✅ Sleep quality
✅ Previous experiences
✅ Activity levels
✅ Fear and anxiety about movement

That’s why two people with similar injuries can have very different pain experiences.

And it’s why some people continue to have pain long after tissues have healed.

Pain is a signal.

But it’s not always a direct measurement of damage.

Understanding this can help reduce fear, improve confidence, and make it easier to stay active during recovery.

That doesn’t mean pain should be ignored.

It means pain should be understood in the proper context.

The more we understand pain, the less power it often has over us.
