That fear is real.

But hospice does not mean no one cares anymore.

Hospice means the focus shifts to comfort, dignity, symptom control, caregiver support, and quality of life when advanced illness is progressing.

A hospice conversation does not force a decision.

It helps families understand what support is available before everything feels urgent.

Symptoms may become harder to manage.
The patient may become weaker.

Sometimes the hospital stay ends, but the real work starts at home.

New medications.

Weakness.

Confusion.

Oxygen.

Wounds.

Fall risk.

A family trying to figure it all out.

Skilled Home Health may be appropriate when there is a skilled need, provider involvement, and eligibility.

Hospice may be worth discussing when advanced illness is progressing and the goals of care are changing.

The question is not just, “Are they home?”

The better question is:
Are they safe now that they’re home?

A fall matters even when there is no major injury.

It may be the first visible sign that something has changed.

Maybe she is weaker.
Maybe her balance is worse.
Maybe medications changed.
Maybe she is scared to shower now.
Maybe she is doing less because she does not feel safe.

The question is not only, “Was she hurt?”

The better question is:
Why did she fall, and what support does she need now?

Depending on the situation, the next step may involve provider follow-up, skilled Home Health, therapy, hospice education, or additional supportive care.

STEADI, Stopping Elderly Accidents Deaths and Injuries, shares vital resources to prevent falls and improve health outcomes. STEADI recommends three key elements to preventing injury; screen patients for fall risks, access modifiable risk factors, intervene to reduce risk by using effective clinical and community strategies.
