For decades, the pinnacle of healthcare leadership meant scaling hospital bed utilization, managing insurance reimbursements, and optimizing reactive "sick-care."

Today, the paradigm is shifting.

As CourMed® | The Private Health Office Chairman Derrick L. Miles, FACHE was recently honored as a University of Alabama at Birmingham Blazer of Influence, a critical question is emerging across health governance:

Why are family offices and UHNW principals managing portfolio risk with institutional precision, while leaving their most vital asset—human capital—entirely unhedged?

In my latest article, I explore why healthcare’s top administrative minds are asking questions about a career in private health security—and how engineering Health Security Infrastructure delivers zero executive downtime, cognitive stamina, and biological protection for key leaders and estate operators.

For family offices, physical health remains the single most unhedged asset in the portfolio.

While family office executives meticulously coordinate cross-border tax structures, private aviation, physical security, and estate planning, health logistics are far too often left fragmented.

We are seeing a massive shift at the macro level:

Gulf Cooperation Council (GCC) Leadership in Longevity: Institutional frameworks are rapidly formalizing across the Middle East. Abu Dhabi’s Department of Health recently licensed the Institute for Healthier Living Abu Dhabi (IHLAD) as the world’s first licensed Healthy Longevity Medicine Centre, focusing on precision lifestyle medicine, longevity research, and innovation to proactively address aging. Meanwhile, in Dubai, His Highness Sheikh Mohammed bin Rashid Al Maktoum issued Law No. (17) of 2026 on June 10, 2026, establishing the Dubai Longevity Authority (DLA). The DLA is tasked with positioning Dubai as the world’s leading hub for regulated longevity, wellness, and advanced healthcare, directly supporting the Dubai Economic Agenda D33 and Dubai Social Agenda 33. Preventive health and longevity are moving from discretionary luxury into sovereign, regulated systems of care.

The Macro-Economic Imperative: A recent World Economic Forum report, The Longevity Dividend, emphasized that physical health and financial resilience are deeply linked—finding that early, preventive interventions alone could unlock multitrillion-dollar health savings and global productivity by 2040.

The Gap for Globally Mobile Families: Many high-net-worth families rely on episodic wellness retreats or high-end diagnostic clinics. But a weekend at a retreat in Europe doesn't solve what happens when a principal faces a health event in New York, London, or Dubai.

Without a structured health continuity framework, health records, emergency protocols, provider access, and decision-making authority remain scattered across people and jurisdictions just when the family needs them most.
A sudden acute health issue or cognitive decline isn't just a personal matter—it carries real operational, business leadership, and succession risks for the family enterprise.

In my latest thought leadership article in Crain Currency, I break down how family offices can integrate health continuity directly into their broader governance strategy without stepping into the role of directing care.
